Audio problem: Despite extensive overdubbing in the scene where Bob returns, the roar of helicopters can still be heard in the background. According to producers on the DVD commentary, the noise was due to firefighting aircraft overflying the sound stage during devastating L.A. brush fires in the Fall of '06. (00:25:30)
Audio problem: When Hawkins asks Jake if he needs help putting out the third fire, he uses his hand to stop Jake from opening the car door a couple of times. The first time he does this, the thump of his hand is heard a split second before his hand connects with the door.
Audio problem: When Lita and Drake get out of the car, each of them closes just one door. But we can distinctly hear three car doors slamming shut. (00:15:00)
Audio problem: The keys we see being depressed on the player piano never bear any resemblance to the actual notes we hear. (00:13:00)
Audio problem: In the "Pilot" part 1, when Hanson is chasing the car we see him on his CB, yet in the shot of the car the driver's hands are on the wheel, but you can still hear Hanson talking on the CB.
Self Defense - S3-E7
Audio problem: As the Stone and Schiff are watching the news report on the security camera tape the reporter has a voice over going whilst the tape is playing. Towards the end of the tape she is shown on TV and is doing a report on the case. You can see her lips don't come close to synching up with what she is saying. The piece was already airing on TV, so very doubtful the TV station missed the large discrepancy.
Audio problem: When Foreman comes to inform the patients parents that the surgery to relieve inter-cranial pressure was successful, the mother asks "What happened? Did she hit her head at a meet or...?" The coach's audio responds with, "No, nothing like that," but the words don't correspond to his mouth movements. (00:31:00)
Audio problem: When Gunny's sister shows up at JAG, Gunny's mouth doesn't move when he greets her. (00:08:45)
Audio problem: When everyone is leaving the restaurant and walking down the street, some are going off for a night cap, Rosemary says she can't go because she has to work tomorrow. But her words and her mouth are out of sync, as are the others to a lesser degree.

Audio problem: Benson and Stabler are searching Darrel Guan's room. When Stabler picks up the bloodstained pants, Benson 'says', "blood" but her mouth doesn't move. (00:28:05)

Audio problem: When Daphne takes off in the golf cart after picking up the rest of the gang, it makes the sound of tires on pavement. But they are on the dirt floor of the Coliseum. (00:08:55)

Audio problem: When Abby tells McGee to shut up near the end, the audio is out of sync with her mouth. (00:34:40)
The Foot in the Foreclosure - S5-E8
Audio problem: In the beginning of the episode, Booth introduces his grandfather to Sweets and his grandfather asks, "Where did you get your M.D., in a Cracker Jack box?" You can hear Bones laugh, but her mouth remains closed.
Audio problem: There's a bit of a mix-up in the codenames of the cars who chase Reggie Dyer. Unit 7 is the first to give chase and it is identified by its crew with the photographer. Then Unit 8 intercepts at the rendez-vous point. Check the license plate; FVW 49. Japp mentions a "unit 10" that is not shown. Later in Chinatown, Japp calls for the other cars, and "Car 8" responds, but its license plate is now YD 8501 and it's another car model. Unit 10 is again not shown. They dubbed in the wrong voiceover, they should have kept offscreen the second response by Unit 8 and made it Unit 10's.
Audio problem: When Hardison is talking to Carrey, just after he names his price ("30%"), there is a shot from a strange angle in which we hear Hardison talk while his mouth doesn't move at all.
In the Public Interest - S14-E3
Audio problem: There's a scene where Frost and George are in the car. Frost's phone rings and you can hear both the ring tone and sounds of the phone vibrating against a surface. The problem is that when George is holding the phone in his hand it is still making very loud vibrating sounds, like it would if it was still lying on a surface. When you hold a vibrating phone in your hand it doesn't make noises like that.
Audio problem: When Oliver is fighting the guys in the hallway, he shoots the second to last guy. But when he starts to draw an instant beforehand, you can see that his right hand, with which he holds the arrow, is nowhere near the quiver when the sound of the drawn arrow begins.
Audio problem: Shaggy and Scooby are walking around trying to find the Ape Man to lure him into the trap. There is a sound and Shaggy says "What's that noise?" It cuts to Scooby who is following him who says "RI ron't row." But his mouth does not move at all when he says this. (00:14:15)
Any Old Port in a Storm - S3-E2
Audio problem: Pay phones always dropped your coin into an internal cash box when you finished your call. In the bar, Columbo makes two calls, and after the first, we hear the coin drop as it should. When he finishes the second call, we should hear that coin fall, too, but it doesn't. (00:34:05)

Audio problem: When Tintin walks into Sakharine's apartment, he says "Mr. Sakharine". His mouth doesn't move when he says it. (00:06:30)
