Revealing mistake: When Kate is shot by Ari, Michael Weatherly flinches before the sound of the gunshot, in anticipation of the forthcoming squib explosion. (00:42:20)
Visible crew/equipment: While Ducky starts to tell Jimmy about an English earl, when Gibbs walks into Autopsy, the reflection of crew members are visible on the steel door's surface. (00:08:35)

Continuity mistake: When Gibbs has the Mafia boss against the car, his arm is across the Mafia guy's chest. Camera cuts and now it is right next to his neck. (00:34:50)
Continuity mistake: Just after Fornell is arrested, the agents are showing NCIS their surveillance of the mafia. When doing so, the headset on the one agent changes position instantly. (00:22:00)

Continuity mistake: When Aaron Wright lights his cigarette in the NCIS elevator, he takes it out of his mouth to blow out a puff of smoke. His arm remains down by his side, but in the next shot his cigarette is instantly up in his mouth. (00:29:10)

Character mistake: In isolation, Tony mentions John Travolta and says that he's known as the "king of cool." That nickname referred to Steve McQueen, not Travolta. (00:15:10)
Factual error: Abby says she ran X-ray chromatography. There is no such thing. I assume she meant X-ray crystallography.
Visible crew/equipment: On the rooftop, after Caitlin leaps in front of Gibbs and takes the bullet in her vest, just as Gibbs and Tony help Caitlin stand up, in this shot the blue crash mat is visible on the ground. (00:41:45)

Continuity mistake: While Caitlin and Tony are at the hospital in Jeremy Davison's room, when Michelle walks in there are two closeups of Jeremy handcuffed to the bed rail. In the first closeup the handcuff is around his wrist with the double bar facing up, but in the second closeup as Caitlin uncuffs him it's the opposite way, with the single bar facing up. (00:27:55)

Visible crew/equipment: When Gibbs joins Caitlin on the beach just as she finishes up her call, in the wide shots their two reddish T-marks (plastic type) are visible on the sand, where each of them stand. (00:06:20)
Visible crew/equipment: After Caitlin shrieks and tells Abby that Gibbs is driving, Abby replies that she's saying a prayer in many languages. It then cuts to a shot of the car Tony's driving, and the reflection of the camera comes into view on Tony's window as the filming vehicle pulls alongside. It's visible again in a following shot. Visible crew/equipment: While Gibbs is interrogating Cluxton they are alone in the room, and when he shows her Harlan Wilson's death certificate, in the overhead shot as she begins to snap, the leg of a crew member is visible on the right side of the screen. (00:42:05)
Visible crew/equipment: While Ducky and Gibbs are at the crime scene in the field, in the long shot just as Ducky says, "He took a tumble down the hill," the raised camera crane, camera, and camera operator (feet are dangling off the front of the crane platform) are visible at the left side of the screen.
