Paper Work - S6-E11
Continuity mistake: When John's in the storm drain with the boy, in the closeups the water rushing from the broken hydrant has a very strong current, and Roy's holding the boy's head up out of the water, but in the wide shots the current is nearly calm, nowhere as intense as the closeups, and Roy's not holding the boy's head.
Paper Work - S6-E11
Revealing mistake: At the start of the episode, when the camera zooms in on the words 'Los Angeles County Fire Station 51' which are attached to the side of the building, we can see that this shot is actually played in reverse - notice the cars/trucks behind the station are driving backwards. Additionally, note that the real 127's rig is seen in the driveway to the right. Also interesting to notice are the upper/lower metal bars which are attached to the bricks, where we can see the bars are much longer after 'Station 51', with a space for something missing, due to the fact that is was made to hold '127' on it.
Paper Work - S6-E11
Continuity mistake: When Johnny and Roy are with the boy who is stuck in the storm drain, note that the shoes both paramedics are wearing are flat bottom, dark grey, water-type shoes, and not their regular black uniform shoes. Then, when the boy is lying on the grass, Johnny's in his uniform shoes, though Roy is still in those water-type shoes.
Paper Work - S6-E11
Continuity mistake: When the car swerves to miss the dog and hits the fire hydrant, the area of ground around the front tire is bare, but in the closeup as the man and his son exit the car, that area is now covered in vegetation, enough to cover half the tire.
Paper Work - S6-E11
Continuity mistake: When the new supply nurse gives Roy the box of supplies, in his right hand Roy holds the receipt and supply box, and in his left hand is the handie-talkie, but in the closeup Roy holds the supply box with his left hand and a bag of saline in his right hand.
Loose Ends - S6-E12
Continuity mistake: At the freight yard when the two men are talking to each other after the Montana man says that the operator has been a good friend, the front of the boxcar is seen, and some fire vehicles (a battalion car and a deluge) are visible on the left of the screen. When Engine & Squad 51 come on the scene, those fire vehicles have disappeared. (00:33:55 - 00:36:05)
Loose Ends - S6-E12
Continuity mistake: When DeSoto unbuttons the police officer's shirt, one of the clear, round, self-adhesive pads is already on his chest, before DeSoto places any of the ECG electrodes.
Loose Ends - S6-E12
Visible crew/equipment: At the hospital, when Gage sells the picnic tickets to Dixie and Dr. Early, just as Dr. Brackett walks up to the nurses' station, both Brackett's and Johnny's T-marks are visible on the floor.
Loose Ends - S6-E12
Visible crew/equipment: When the guys bring Brackett to the hospital after his car accident, two white tape, and one black tape actors' T-marks are all visible on the floor in the ER.
Loose Ends - S6-E12
Other mistake: While Brackett is a patient at Rampart, he phones to inquire about Tina, and when he rips off his hospital ID bracelet we can see the wood grain bathroom door as he heads for Tina's room. When he's in Tina's room, the wood grain bathroom door in her room is identical to the door in Brackett's room, revealing the same room is used for both patients.
Loose Ends - S6-E12
Visible crew/equipment: During the fire at the train yard, when they carry out the second toxic fume inhalation victim from the boxcar, just before they pass the camera, the shadow of the camera tracking backwards is visible on the boxcar, to the left.
Loose Ends - S6-E12
Other mistake: At the fire in the freight yard, Captain Stanley asks the dispatcher to respond a second-alarm assignment, and then we see Engine 8 pulling out of their apparatus bay and driving down the street. The problem is that this particular shot of Engine 8 is from season 5, episode "Equipment", when Johnny was doing some overtime at Station 8, and we can actually see Johnny sitting on Engine 8.
Loose Ends - S6-E12
Visible crew/equipment: When Dixie visits Brackett in his hospital room, he insists on seeing the little girl from the accident, but she convinces him to get back into bed. Then as he says "Alright, but later I want to see her," the actor's tape mark is visible on the floor near the second bed, where Dixie stands as Brackett gets back in bed.
Loose Ends - S6-E12
Continuity mistake: When the police officer has chest pains, DeSoto places the ECG electrode discs onto his chest. In the close-ups, the right disc has a black lead head, but otherwise the right disc has a white lead head.
Loose Ends - S6-E12
Continuity mistake: When Johnny and Roy, who are off duty, show up at the nurses' station so Johnny can sell some more tickets, note the paramedic status board on the wall in the base station room. In the closeups 51's red and yellow magnets show that both the engine and squad are at the station, but in the wide shots both magnets are in the other columns.
Loose Ends - S6-E12
Continuity mistake: At the freight yard fire, after the two men with toxic fume inhalation are located and rescued from the boxcar, Battalion 14 arrives and we see that the Battalion Chief is McConnike, and Captain Stanley even calls him "Chief." The problem is that it's not until episode 6x14, "Insanity Epidemic," in which McConnike is made the Battalion Chief, and that causes Cap to act a bit like a paranoid nut due to an incident years before. Episodes were often aired out of the order of which they were filmed. As an aside, when McConnike arrives, watch him trying to put on his turnout.
Loose Ends - S6-E12
Continuity mistake: When Roy and Johnny are driving to the hamburger stand and stop to have a look at the squad's fanbelt, in the interior shots Roy is wearing his watch while driving, but when he exits the squad the watch has vanished.
An Ounce of Prevention - S6-E13
Visible crew/equipment: When Roy and Johnny are on Tom Jensen's show, the outline of Johnny's lavalier mic transmitter is visible on the front, right of Johnny's shirt, over his belt, as he and Roy begin their demonstration, and also after Jensen's crew member electrocutes himself when Johnny and Roy carry and lay him down near the table. This has nothing to do with the wired mics that are clipped onto Roy's and Johnny's ties for Tom Jensen's show.
An Ounce of Prevention - S6-E13
Continuity mistake: While John and Roy begin rehearsing in front of Marco and Chet, when John lights the flame his watch reads 10:00, but just a few minutes later when John puts the sawdust into the strainer his watch reads 10:30. When Squad 51 is dispatched to "sick child," timeout is 12:13, and after John and Roy get back to the ER the second time after searching the garage for the poison it's 1:05 at the ER. Then when they're on the Tom Jensen Show, as Roy tries to light the flame John's watch reads 11:30, but when he and Roy are treating the electrical shock victim, his watch reads 4:00.
An Ounce of Prevention - S6-E13
Continuity mistake: After Early has examined the woman who was stuck up on the ferris wheel with her husband, he tells her she could go to the waiting room, and when Early leaves treatment 2 the stethoscope is around his neck, but next shot from the hallway as he exits the stethoscope is gone.
