The Night of the Brain - S2-E21
Revealing mistake: Mr. Braine fires a mini-rocket at Brendage, killing him. But a poor camera angle reveals the explosive charge going off in the wall more than two feet to Brendage's left. From the angle at which it was fired, the rocket couldn't possibly have hit him. (00:21:45)
The Night of the Brain - S2-E21
Continuity mistake: West is lying down while in hand shackles. When Voulee turns her back to him, he releases a gadget from under his sleeve by his right wrist. While the camera closes in and out of the device to free him, West's shirt sleeve by his wrist keeps appearing and disappearing. (00:38:35)
The Night of the Brain - S2-E21
Revealing mistake: Mr. Braine screams as his steam-powered chair crashes into the bandstand. If you look closely at the next shot, just before the explosion, you can see that the dummy now in the chair has its head bowed forward. It also has light brown hair, rather than Mr. Braine's silver hair. (00:47:10)
The Night of the Brain - S2-E21
Continuity mistake: The opening of the last scene is of the personal train car in the dark night with very little moonlight. The closing scene of this episode shows the train again amid the night sky which is a much lighter beautiful blue. (00:47:45 - 00:49:15)

Revealing mistake: When Gordon is looking outside on their train car platform, it is obvious the background desert scrub is a painted scenery flat.
The Night of the Brain - S2-E21
Other mistake: Throughout this episode Gordon keeps producing orange-sized smoke bombs, but he has no pockets in his clothing large enough for all of them and if he did, the size of the bombs would be obvious in his clothing.
The Night of the Deadly Bubble - S2-E22

The Night of the Deadly Bubble - S2-E22
Continuity mistake: When West and Dr. Pringle back up to the steel door to escape, West is about three feet in front of it. One of the villain's henchmen bursts in, saying, "hold it." The next shot and West is about eight feet away from the door and the henchman.
The Night of the Deadly Bubble - S2-E22
Continuity mistake: Dr. Pringle and West back up to a steel door to escape from the room and West is behind a table with lab equipment on it. A henchman charges in, gun drawn on West. The next instant shot shows the table has moved several feet to the viewer's left, presumably to make room for the impending fist fight West will have with the bad guys.
The Night of the Surreal McCoy - S2-E23
Continuity mistake: During the fight, the candle on the left tilts to the left. Next shot, it is perfectly vertical. West knocks a thug into the bureau, and left candlestick tilts to the right. After the fight is done when West is opening the door for Gordon, the candle is inexplicably on the floor. (00:12:45)
The Night of the Surreal McCoy - S2-E23
Continuity mistake: The second thug to be knocked-out is on the floor (to the left of screen) and has fallen on his left-side with his left leg slightly bent, and his right leg cannot be seen. When Gordon enters the room, this unconscious thug is now positioned face down, arms extended over his head, and both legs stretched out. (00:12:55)
The Night of the Surreal McCoy - S2-E23
Continuity mistake: The last thug to be knocked-out is on the floor by the fireplace with his body at an angle to it. When Gordon enters the room, this unconscious thug is closer to the fireplace, and his body has changed position to be parallel to it. (00:12:55)
The Night of the Surreal McCoy - S2-E23
Continuity mistake: West enters the portrait and is stretched out on his back on the ground. As he is walking to the saloon, his entire backside of his suit is covered in dirt. West becomes dizzy, and as he is falling to the ground, face first, we see that the entire back side of his suit is clean. (00:35:50)
The Night of the Surreal McCoy - S2-E23
Continuity mistake: When West is pulling the string to shoot the gun, we can see under both knees his pants are covered in dirt. As he runs to enter the Saloon, the front camera shot shows no sign of dirt. (00:44:10)
The Night of the Colonel's Ghost - S2-E24
Continuity mistake: The opening scene of this episode shows the train moving at twilight. West and Gordon are looking out the window, and it is pitch black. Right before West goes outside, the camera shot of the train wheel shows light (either twilight or moonlight) on the wheel. The very next scene has West with the train door open, and it is pitch black outside. (00:00:05 - 00:00:35)

Continuity mistake: When West ties his horse to column, there is a boarded up entrance to the left with one wooden plank at a 45 degree angle in the middle. When West returns to his horse to leave, the plank is missing, and West rides off without having to untie his horse. (00:03:05 - 00:05:30)
The Night of the Deadly Blossom - S2-E25
Continuity mistake: When Barclay hangs the bone hatchet back in its place, the outline on the wall supposedly left by the item differs significantly from the artifact itself. (Most noticeable is the upper end: the outline has one long curled tip, but the actual hatchet has two short prongs there.) The outline was apparently drawn without the prop, or from a different prop altogether. (00:12:05)
The Night of the Deadly Blossom - S2-E25
Plot hole: There are two rockets to be launched. The right rocket launches. It is important for the plot for the remaining rocket to explode without being launched. Without explanation, the left rocket just falls off its ramp, lands on the floor and shortly explodes. (00:46:10)
