The Night of Sudden Death - S1-E4

Continuity mistake: In the scene after the first freeze frame, Corinne is affixed on a round arrow target. Corinne's body keeps changing positions even though the target has stopped spinning. First long shot, she is horizontal with her head at #3. The close-up has her almost vertical with her head at #1. Next close-up, her head is at #2. Following long shot, she is horizontal (head at #3). This keeps occurring throughout this scene.

The Night of the Glowing Corpse - S1-E7

Continuity mistake: Near the end of the episode when West walks into the mansion where the meeting is behind held, just as he enters, Lt. Armand Renard can be seen talking to Artemus at the bottom of the staircase, but Renard died earlier in the episode. They obviously filmed this scene at the same time as the first scene, but missed that continuity error in the editing. (00:46:40)

The Night That Terror Stalked the Town - S1-E10

Revealing mistake: During the final scene where West and his double, Janus, are shooting at one another, the two look identical to one another. West goes to the roof and jumps off onto Janus who is by the cannon. However, the revealing mistake is the original looking Janus is playing the role of West diving off the roof, and the one who is fighting Robert Conrad who is supposed to be playing his double, Janus, is standing by the cannon. When the two begin to fight on the ground, one should not be able to tell the two apart, but it is apparent the faces are different. The mistake is fixed when the two get off the ground and now both fighters look like West.

The Night That Terror Stalked the Town - S1-E10

Continuity mistake: Three fourths into this episode, West is in a room with an hourglass. The hourglass shows a few grains of sand before it empties. Next shot is of Gordon trying to get through the chained gates. Camera shot back to West's room, which shows a fuller hourglass with more sand, which lasts for minutes as this episode continues, until all the sand empties from the top.

The Night of the Red-Eyed Madmen - S1-E11

Continuity mistake: The opening of the last scene shows the train moving through an area with large trees along both sides of the track. The closing scene shows the train in an arid area with uneven rotting wooden posts and dried up sage grass.

The Night of the Human Trigger - S1-E12

Revealing mistake: In the next to last scene of this episode during the Sawtooth earthquake, while boulders are falling and trees are being uprooted, West, Gordon, and Faith are not struggling to keep their balance so as not to fall, they are just motionless standing - something that cannot happen with such a strong earthquake. The camera, which is shaking, attempts to make it look as if the three are shaking with the earthquake.

The Night of the Human Trigger - S1-E12

Revealing mistake: In the opening earthquake scene right before the freeze frames appear, West and Gordon are in the bar. The glasses and pitcher on the bar by the bartender, the bottles of liquor behind the bartender, the bottle on top of the piano, and the ashtrays on two tables never move out of place, even though other tables have broken, chairs have toppled, the ceiling is destroyed, and the support beam falls. It is obvious the camera is doing the shaking, not the ground.

The Night of the Infernal Machine - S2-E14

Trivia: West collides with the cook and sends him sprawling face-down in a huge platter of gelatin. Before racing off again, West quips, "Sorry about that, chef!" This was a winking homage to the 60s spy comedy Get Smart and its bumbling hero Maxwell Smart's famous catch-phrase, "Sorry about that, Chief!" (00:47:30)
