Continuity mistake: In the final scene, Gibbs is working on his boat wearing safety glasses and has a pencil behind his right ear under the frame. When he learns Sergei has survived his pulls his glasses off in disbelief and the pencil is no longer behind his ear. After a cut away to the director and back to Gibbs again, the pencil has returned and remains there for the rest of the scene, including the black and white closing transition.
Visible crew/equipment: When Tony is with Captain O'Rourke in the parking lot, Borin approaches from behind. As O'Rourke turns to face Borin the actor's microphone has slipped out from under his shirt, between the buttons. Then when Borin says, "So will you," in the next shot facing O'Rourke and Tony, that mic is visible again. (00:38:10)
Factual error: The entire episode is contrived, as the Navy corpsman who is in legal trouble for rendering aid in an accident would not be in trouble due to the "good Samaritan law" in force in Virginia at the time.
Continuity mistake: When the Russian agent is talking to the bad guy, the Russian agent's coat collar changes position in just about every shot.
Continuity mistake: When Gibbs meets with Tom Morrow at the dinner, Tom asks Gibbs if he is going to upgrade to a smart phone. Gibbs opens his phone, the shot changes and his phone is now closed. (00:15:15)
Visible crew/equipment: After Jimmy and Tony's chat, when Tony heads to the elevator Jimmy mutters that he hates being out of the loop. In the next overhead exterior shot of Tony's car, on the rear passenger window we can see the reflection of the boom operator walking and holding the boom pole/microphone. (00:21:30)
Visible crew/equipment: When Gibbs and Tony are in Vance's office, after viewing Abby's S-mine video Vance walks over to his desk, and the two green tape marks are visible on the floor where he stands. (00:26:25)
Continuity mistake: The banana peel on DiNozzo's index finger, healing his owie, moves to his thumb in one shot. (00:12:00)
Revealing mistake: When Abby and Palmer tell Gibbs about General Gadet, Palmer notes that Ellie has been trying to find his last location, and Ellie replies, "Which I think I have and something more." Then Ellie walks over to Gibbs with her laptop to show him the NSA intel report about General Gadet. The problem is the first sentence of the NSA report reads, "Which I think I have and something more," which was Ellie's reply in the dialogue, and a few lines down are three paragraphs of nonsensical filler text. Revealing mistake: While Ellie and George are in the attic, Ellie uses her laptop to access contact info and gets Richard Doogan's DOD military service record. There's a closeup of the laptop screen as she begins to read aloud, but the problem is part of the paragraph reads, "And he's only a few hours away in Kansas..." and especially "I need to update Gibbs" which are Ellie's lines in the dialogue with her brother. Visible crew/equipment: While Gibbs and Alex are in the stateroom with Kasey and Curt, when Kasey tells them about the polyps on her vocal chords, the microphone can be seen hovering in front of Kasey, at the bottom of the screen. (00:28:45)

Other mistake: While panning the front of Gibbs' home, the roof of one vehicle and the windshield/passenger mirror of a second can be seen behind the gate/fence in Gibbs' driveway behind his truck. He lives alone and does not share a driveway with other homes. In reality, Gibbs' home is located at 2647 S. Magnolia Ave in LA, and the vehicles likely belong to the real residents/homeowner. Visible crew/equipment: After Gibbs refuses to make a deal with Triff, just as Gibbs and McGee exit the prison's visitor cell, the shadow of the boom operator holding the boom pole/mic can be seen on the walls behind them. (00:32:30)
