Forest of the Dead (2) - S4-E9
Continuity mistake: Near the end of "Silence in the Library," a sweeping shot shows that they're standing between two freestanding bookshelves. But at the beginning of this one, when they're supposed to be in the same place, they're able to break through a wall into a separate room.
Revealing mistake: In the opening scene the Doctor and Donna are speaking over the phone. The purple phone Donna is holding shows the slot where a cable should be plugged in, but there is none. (00:00:25)
Suggested correction: This shouldn't be considered a mistake. While it's true in real life this phone has a slot for a cable, they're on an alien planet and this is unknown technology. In-universe there's no way of knowing what that slot is for, and there's no reason to think the phone needs to be plugged into anything.
Continuity mistake: After the TV signal cuts out due to the crash of the Titanic, when Sylvia says "It's gone dead" while searching for a channel, she moves her arm with the remote forward the same as she did in the previous, wider shot. (00:18:57)
Revealing mistake: When the Doctor and Donna run back inside the TARDIS to find the console room lit up in red and the Cloister Bell ringing, what's visible outside of the doors as they run in isn't the bright Shan Shen street the TARDIS is parked on but the dark soundstage the set was built in. (00:47:49)
Plot hole: In this episode, the Doctor is said to have died during the events of "The Runaway Bride." As a result, he was not able to prevent the spaceship Titanic from crashing in London. It is later shown that southeast England was obliterated by the explosion. This falls far short of what the Doctor mentioned in "Voyage of the Damned", wherein he frantically repeated that if the Titanic crashed, the drive's explosion would wipe out all 6 billion people on the Earth (that's the entire human race at the time of the show's airing).
Continuity mistake: In this episode, Rose's teeth look different than in all her other appearances. She also has a different accent, both for no apparent reason.
Continuity mistake: The house that Donna and her family are shown living in in Series 4 is different from the house the Doctor dropped her off at at the end of her first appearance, "The Runaway Bride." You would assume they'd merely moved - except that in this episode, Donna and her family are shown living in their Series 4 house before the events of "The Runaway Bride", on the day Donna gets the job that leads to her meeting the Doctor in the first place.
Continuity mistake: Before Martha uses Project Indigo she doesn't have an ear piece. A shot later when she says, "Bye, Jack", she does (and then takes it out). (00:17:56)
Audio problem: When Martha is talking to everyone on her laptop, we can see her mouth moving from the front view. Yet when the camera cuts behind her, she's still talking but we don't see her mouth move. (00:28:32)
Revealing mistake: When Torchwood is transmitting a signal to the Doctor, there is an overhead shot of Cardiff in which several shoppers are idly walking through apparently deserted streets. (00:31:55)
Continuity mistake: When the TARDIS is breaking through the time limit to reach the stolen planets, the console room catches on fire from the strain. But after reaching its destination, the console room is abruptly back to normal. (00:35:05)
Continuity mistake: In the scene toward the end where the Doctor is shot by the Dalek, when the camera is shooting from Rose's point of view, the Doctor is gasping and struggling for breath. However when the camera shows the Doctor from behind, he's lying completely still in Rose's arms. The camera cuts back to Rose's POV and he's struggling again, with no time in between for the change in his movements. (00:43:35)
Continuity mistake: At the end, when the Doctor is watching his hand starting to glow, shots from his point of view show that he's holding it vertically. But shots from the opposite view show that he's holding it horizontally. (00:43:39)
Continuity mistake: When the Daleks capture the TARDIS in the chronon loop, the TARDIS is the right way up. But when it is being pulled towards the Crucible, it is upside down. (00:06:22)
Continuity mistake: When the Doctor, Rose and Jack get out of the TARDIS onboard the crucible, the console room is lit in very blue light. But in the shots of the console room before and after, it's lit completely differently. (00:09:15)
Continuity mistake: When Martha arrives at Osterhagen Station 1, she unbuckles the belt of her teleport backpack, but in the wide shot immediately afterwards the belt is still buckled. In the next shot, it's back to being undone as she takes it off. (00:18:47)
Continuity mistake: When Davros activates a screen in the Vault to show the Doctor and Rose the Reality Bomb test, the screen shows the prisoners being herded into the testing area, something that already happened shortly before. (00:23:14)
Plot hole: Near the end of the episode, the part-human Doctor is said to have all of the memories of the original Doctor. If this is true, then the part-human Doctor should remember the time the original Doctor spent with Rose, so shouldn't commit genocide or need Rose to help him to not be a man of killing.
Plot hole: Davros said that the reality bomb would destroy all parallel universes. He fails in the Doctor's universe, but the Doctor said in an earlier episode that every decision creates a new parallel universe, like if Donna had walked out of the TARDIS with Jack, Rose, and the Doctor. She wouldn't have imbued herself with Time Lordness, so Davros would have succeeded. This means that all the universes, including the Doctor's one, should have been destroyed, because if the Reality bomb only succeeds in one universe, it would destroy all the others as well.
Continuity mistake: When the Doctor grabs the rope there are several metres more behind him. But, as he's hoisted off the ground, he is almost at the end. (00:03:34)
