Continuity mistake: Stottlemeyer has taken things like a banana and a Monkey Joe out of a black leather bag and places them on a table while interrogating Darwin the chimp. One shot while he's acting like a monkey shows the table's bare and the bag's closed.

Mr. Monk and the Blackout - S3-E3

Deliberate mistake: As Monk is using the night vision goggles in his house the lights suddenly come back on and continues on still thinking that the intruder cannot see him. The light coming on should have blinded him or at the very least he should have been able to tell there was more light than before even if he hadn't had much experience using the goggles. All the lights in the house come back on and the amount of light would be too noticeable with night vision. This would have been deliberate as Monk continues to think the intruder still doesn't see him in the dark and is part of the joke in the scene.

Plot hole: While convenient to the plot, the computer system used by the medical examiner is far too poor to be taken seriously for a high level agency. While Monk is unaware of deleting the files on the computer, why would there be a one button push to delete all the files on working cases? Even to not have some sort of password to delete files is questionable, as most systems in offices and workplaces will only have functions like this for administrators, executives etc. We find out that all the files are backed up and are slowly being recovered. However, these types of systems are primarily for emergency scenarios such as fire, flooding and so forth. With the ease that Monk is able to delete the files by accident, it seems a bit hard to believe how easy it is to do.

Factual error: As Monk is removing the crumbs off the keyboard, he flicks one crumb away from the backspace key. It gets flicked up and lands near the 0 button on the numeric keypad. Yet when this happens, the screen then says "Do you want to delete files?" as if he hit the delete button.

Continuity mistake: In the opening scene, the orange-and-white scarf that Paul Harley uses to blindfold Lorissa changes positions and color patterns repeatedly between shots as he escorts her from the pool to the garage.

Plot hole: In this episode the police are using the toupee of the commissioner to identify the torso as the missing woman. In reality that would not be possible since cut hair is missing the root and therefore only contains mitochondrial DNA, which can only be used to identify a human's relation via the female lineage, as it is the same in relatives and passed through generations from the mother to her children. Furthermore, the identification of said torso would likely not be sufficient proof that Mr. Harley killed her as he may claim that somebody misled him to believe she was in Paris by placing the telephone call he mentioned to police.

Continuity mistake: In the scene where Monk meets Salvatore Lucarelli (Philip Baker Hall), Lucarelli is cutting up fisheads on a cutting board. Throughout the scene the fish on the table constantly change between shots. One of the notable changes is when Lucarelli is discussing the case he has for monk there is a shot of a fish, and when the shot changes you can now see the fish's head has all of a sudden been chopped off.

Mr. Monk and the Girl Who Cried Wolf - S3-E6

Continuity mistake: About midway through, the new nurse is sitting on Monk's kitchen counter eating a sandwich. She puts the sandwich down on a plate as she hops off, the shot switches to another angle to include Monk as she walks away, and the sandwich has suddenly jumped off the plate and is now lying on the counter a few feet away. Neither of them had time to pick it up or move it.

Continuity mistake: When the episode begins, the Employee Of The Month is shown walking into Mega-Mart. The exterior of the store is shown as a cement color and there is a garbage can right beside the front door of Mega-Mart. When the Employee Of The Month is shown opening up the door to the store, just seconds later, you will see the exterior of the building is now yellow and white and there is no longer a garbage can beside the door. Clearly they are using two completely different buildings. (00:00:40)

Question: Why would it have been political suicide if it was discovered that Mayor Nicholson was secretly meeting with Cusack to end the garbage strike? If anything, if the people found out that they were meeting to find a way to end the garbage strike, wouldn't that have made everybody happy considering how much garbage was piling up all over the city?

Answer: The two men are trying to work out a secret deal between them without involving the union, which means the workers' interests aren't being represented and defeats the whole purpose of a union. The mayor would lose labor's support and Cusack's union troubles would just be starting.
