Continuity mistake: In this episode, an invitation to Piper is addressed to 1329 Prescott Street. But in episode 1-7: "The Fourth Sister," the Halliwell sisters put up a flyer for their lost cat, and it gives their address as 7571 Prescott Street.
Other mistake: This entire episode the ghost brothers whine that Leo is responsible for them dying and he left them to die, even Leo believes this. It makes no sense. As we see in the flashback even if Leo stayed to help they were blown up moments later and still would have died.
Continuity mistake: While at P3, Leo tells Phoebe that she can't turn into a Harpy just by getting scratched, and when Piper cautions Leo to use discretion Piper raises her left arm to Leo, but in the next shot it's Piper's right arm that's raised. (00:16:05)
Continuity mistake: When Phoebe receives the phone call from her doctor informing her that she is pregnant, she faints and starts to fall to the right. The next shot shows her lying down as if she had fallen to the left.

Visible crew/equipment: After Phoebe tells Cole she's pregnant she leaves in the elevator, and when it cuts to Piper, Paige, and Leo we can see the crew's grey mat on the floor under Piper and Paige, then in two following shots (from the same angle) the red T-mark is visible on that mat, at Paige's feet. Other mistake: Piper tells Page that her doctor told her she probably wouldn't be able to conceive because of scar tissue from blunt force trauma. But Leo has healed Piper, if there was scar tissue after Leo heals, then all 3 girls should be covered in scars.
Suggested correction: They are thrown around quite a bit before even finding out Leo is a whitelighter. Also, he doesn't heal them every time they get thrown or hit in the abdomen. In fact, we usually don't see him heal them at all if they're just thrown across the room or punched in the gut, unless there's a visible wound.
Visible crew/equipment: When Phoebe's assistant walks into the office carrying a tub of mail for Phoebe, the shadows of the boom mic are visible on the wall over her and the TV. (00:00:25)
Other mistake: After they talk to the Angel of Destiny for the first time, they send Leo to ask the Elders if he is legit. After Leo leaves they find out about Agent Jackman, and the following events unfold. After Jackman busts them Piper calls for Leo and tells him "It's OK, you can orb" and he appears asking her "What about Agent Jackman?" Except that Leo was gone before they found out about him and the girls hadn't told Leo yet that he even existed. (00:09:09 - 00:21:58)
Continuity mistake: In the recording that Agent Jackman plays for the sisters, the end of the reversal spell is heard "bring her back to me," but when Paige and Piper said it moments earlier, they only said "bring her back." (00:17:45 - 00:19:10)
Suggested correction: They say "bring her back to me" both times.
Revealing mistake: In the teaser, when Leo levitates to hang a picture, he has a major wedgie and the ankles of his pants hike up to mid-shin due to the harness that is used for the levitation effect.
Continuity mistake: In the scene where Agent Jackman shows them the file he has on them in their dining room, Paige's left dress strap has fallen off her shoulder in some shots, but in others it's perfectly in place.
