The One With Chandler In A Box - S4-E8
Revealing mistake: When Chandler is in the box and Ross accidentally places his hand over the 'air-hole' you can see Ross feeling for the hole for a while before he finds it. (00:12:35)
The One With Chandler In A Box - S4-E8
Visible crew/equipment: Just as Ross asks Rachel for the necklace he got her, a stage light is very visible above the bathroom door. (00:12:35)
The One With Chandler In A Box - S4-E8
Continuity mistake: In the episode with Chandler in a box, when Rachel is showing Ross all the things she had kept from their relationship in a shoe box, her hair is tucked behind her ear in most shots, but when she says "I keep the gifts that matter", it's all loose, then goes back to how it was in the following shot. Also in the shot before that line she's pointing the horn fossil's tip at Ross, but when it cuts to a front view she's holding the horn's tip and pointing the broken end. (00:16:30)
The One With Chandler In A Box - S4-E8

The One Where They're Going To PARTY! - S4-E9
Continuity mistake: When Chandler answers the phone and it's Gandalf calling to cancel, Ross is just opening the fridge, but the shot cuts to Chandler with the phone and Ross is putting two beers down on the side next to him - too quick to have got them there from the fridge. (00:05:35)
The One Where They're Going To PARTY! - S4-E9
Revealing mistake: When Phoebe is looking at the front page of the "Chelsea Reporter," Monica says her restaurant review is on the back page. However, Phoebe doesn't turn to the back page, but merely flips the paper over, looking at the bottom of the front page (and finding the review there). (00:07:30)
The One Where They're Going To PARTY! - S4-E9
Revealing mistake: When Monica and Phoebe are in Central Perk, Phoebe is looking through the list of ideas that Monica has come up with for the use of Phoebe's van. If you look closely (most obvious just after Phoebe says "let's plan the wedding reception") you can see that every single page of the notebook Phoebe is holding has the same thing written on it, instead of lots of different ideas that Monica is meant to have had. (00:17:10)
The One Where They're Going To PARTY! - S4-E9
Continuity mistake: When Joanna stops Rachel from leaving work Rachel's holding her handbag and jacket, and there's still one bag hanging on the wall hook, but when Joanna mentions the "drinking binge" there are two bags hanging from the hook, then it's back to just one bag, yet when Joanna calls Sophie there's nothing on the hook and Rachel has both bags.
The One With The Girl From Poughkeepsie - S4-E10
Continuity mistake: In the scene on the train, when the conductor announces Montreal, Ross's head goes from laying on his shoulder to a upright position from one shot to the next. (00:19:00)
The One With The Girl From Poughkeepsie - S4-E10
Factual error: Ross falls asleep on the train and ends up in Montreal, where the woman he meets tells him "it's only a 2-hour ferry ride to Nova Scotia." Yeah, more like a journey of between 13 hours (12-hour car ride plus 1-hour ferry) and 23 hours (multiple bus legs plus 1-hour ferry.) The character could have been lying to Ross, but it's not presented that way at all.
The One With Phoebe's Uterus - S4-E11
Continuity mistake: At the beginning, Alice and Frank tell Phoebe that they have been trying to get pregnant. When Alice says, "You know, no one's getting any younger", Frank looks at Alice and laughs. In the next shot however, he is looking at Phoebe and doesn't seem to laugh. (00:01:00)
The One With Phoebe's Uterus - S4-E11
Continuity mistake: In the scene where Joey is showing kids around the museum, there's a little boy who corrects Joey about the existence of the Mastodon. The kid's arm is by his side at the beginning of the scene. When he begins saying, "Isn't the Mastodon from the Pliocene Epoch?" his left arm is suddenly up and his thumb pointing at the Mastodon. (00:07:05)
The One With Phoebe's Uterus - S4-E11

The One With Phoebe's Uterus - S4-E11
Revealing mistake: Phoebe already has a visible baby bump before she even agrees to get pregnant.
Suggested correction: It's presented as sarcasm. Not a mistake.

Not clear on where you got that it's presented as sarcasm; nothing in her delivery says so.
