Visible crew/equipment: After Ross and Rachel cover the table with the sheet, when Phoebe walks in Rachel says they can turn out the lights and watch the movie, and in the next shot we can see the end of the set wall, the soundstage and a crew member's leg.
Continuity mistake: When the class is in Arnold's esophagus, Tim says something, then it cuts back to Ms. Frizzle. Arnold is sitting in the back seat of the bus.
Visible crew/equipment: When the son of the accident victims is being led through the trauma rooms to see his parents' bodies, you can see Laura Innes crouching under a table in the background directing the scene. Innes was the director of this episode and it was established that this episode took place on her character's day off. Therefore, she should not have appeared in front of the camera at all. Visible crew/equipment: Approximately 29 minutes and 10 seconds into this episode, right after Sinclair clasps hands with "The One", Garibaldi and Sinclair are on the ground. In the upper right corner of the shot there is a grip with a red baseball cap and red and white shirt on. He has long hair and spiffy glasses.
Visible crew/equipment: Just shortly after Ellen lets Paige in, when Ellen stands at the counter, Paige mentions handcuffs, and then Ellen's boyfriend comes out. As the camera pans to the right, you first see a streak of light in the middle and as the camera continues to move four or five stage lights can be seen. This can be seen on the season 2 DVD set very clearly. (00:09:25)

Audio problem: During the scene where Jim and Owen are trying to bribe the Vicar with chocolates, just after they take them back, you can hear a mysterious voice say "chocolates". The only person who this sounds like is Jim, but in the next shot, his lips aren't moving.
Deliberate mistake: When Johnny is watching the news footage of the Hulk battling Thing, all the footage is merely clips from that portion of the episode, including parts of the battle that were happening before the camera crews even showed up. (00:02:50 - 00:07:20)

Continuity mistake: Connor (is this cool?) Hammil's shirt is unbuttoned, then buttoned between shots (just as he pushes the woman's head through a pane of glass).
Origin of Iron Man: Part 1 - S1-E11
Character mistake: When young Tony Stark is telling his father that their helicopter's anti-flak chip didn't kick in, he acknowledges that it was his fault by saying he blew it. When his father corrects him and says "No Tony, we blew it. Stark and Son," Tony defensively says "The anti-flak chip failed, it's not my fault," completely contradicting what he said seconds earlier. (00:14:00)
Neogenic Nightmare Chapter 11: Tablet of Time - S2-E9
Factual error: When Peter is walking Alissa home, the traffic signal at the intersection they stop at goes from red to yellow to green. Not what US traffic signals do - they go straight from red to green.
Suggested correction: Not entirely true; although they are rare and may no longer exist now, I've seen traffic lights that followed the red, yellow, green pattern as recently as the mid-1990s.
Interesting. What state (s) did you see this in? I'm assuming going from red to yellow was to encourage cars to cautiously enter the intersection in case someone was running a red light?
Traffic lights in the UK do this - it's more to give you a second to get ready, in gear, etc., then as soon as the lights turn green you can go. Otherwise you get no warning of when the lights are about to change.
In Illinois; as I said, such traffic lights are rare, but they did exist at least as recently as the time this episode of the series aired, and they may still possibly exist in larger cities such as New York City.
This traffic light set-up (red to yellow to green) still exists today in the UK. From what I understand, it is to alert the driver that the light will be turning green imminently and to prepare themselves to put their car in gear, as manual cars are still pretty common in Europe. I'd wager this light cycle was phased out of North America due to the abundance of automatic cars today. Could have been different in 1994 though.
It should be noted that traffic lights that go from red to yellow before going green keep the red light illuminated so that both red and yellow are lit up. However, that's not what happens in the scene. I've never seen a traffic light operate the way it's shown. And Massachusetts still has traffic lights that go from red to yellow, however, when red and yellow are lit up together, this allows for pedestrian crossing.
Revealing mistake: When Danziger is rolling down the cliff and almost goes over the top, you can see the safety rope over his left shoulder. (00:28:00)
Visible crew/equipment: When Nina joins Lt. Cooper, they both head upstairs for lunch and fresh coffee. Then, while the camera begins moving toward the front desk (before we see Gina), the reflection of the dolly, camera, camera operator, and boom pole are visible on the glass partition. (00:17:55)
Continuity mistake: A whimpering housemaid rushes down the stairway, frightened by an intruder on the second floor. Doctor Watson gallantly charges upstairs to investigate, plunging his left hand into the right side of his jacket to retrieve his trusty Webley revolver as he advances on the camera. However, the shot immediately cuts to Watson withdrawing the revolver with his right hand from the left side of his jacket. (00:29:55)

Continuity mistake: When Lisa and Ray are talking in Lisa's apartment there is a soda bottle on the table. Part way through the scene a cap appears on the bottle and then is gone in the final shot.
Continuity mistake: When Gersham runs from the police in the first scene, his vehicle crashes on a rooftop and hits a wall, bouncing off it, turning and stopping parallel to it. He exits the vehicle, at the point where he crashed, and the vehicle is now positioned differently (turned 90 degrees, pointing straight to the wall). (00:04:20)

Continuity mistake: When Burke questions him on the golf course, Conrad Hill is completely in shadow for all the frontal shots, but in full sunlight in the over-the-shoulder reverse angles. (00:14:00)
The Accident - S1-E1
Visible crew/equipment: When Alex goes into the garage for the first time, a camera and camera operator can been seen reflected in the saw.
Continuity mistake: To beat the traffic, Coop skateboards inside a construction pipe. The pipe is on the back of a truck, so it's not exactly very long, and gets picked up by a crane arm mounted on the truckbed itself. However, Coop takes ages to come out of the pipe, and in the shot from inside the pipe it looks as if is dozens of meters above the street level, which isn't. (00:08:00)
Factual error: Near the beginning of the episode dealing with the life of Moses, they depict various scenes of ancient Egypt, during his time. But they show the Sphinx with the nose missing (as it looks today), when that face was intact until the early 19th century when Napoleon's troops used it for artillery target practice.
Coach Fussell's Lament - S2-E5
Continuity mistake: Chairface and Chrome Dome arrive at the Brianchild's house in the same car, but drive off in separate cars.
