Visible crew/equipment: When in her office, Grace grabs Chucky, then Chucky stabs her twice. After he stabs her, in one shot she stumbles back, and as she does you can clearly see the head of a crewmember behind her! (00:58:40)
Continuity mistake: When Grace Pool is stabbed, she falls onto the photocopier, and Chucky falls to the floor, beside the photocopier. But in the next wide shot he's gone, with not enough time to of done so. (00:58:40)
Other mistake: Many times in the film, it's obvious that Chucky is inactive and not alive, yet supposed to be. When he falls to the floor beside the photocopier, for instance, it's obvious he is off and inactive. (00:58:40)
Continuity mistake: When Chucky closes the door so Kyle can't get in, he has his arm by his side, but in the next shot when he says 'Ok sport' his hand is up and pointing at Andy. (00:58:55)
Continuity mistake: When Grace falls to the floor dead, photocopies fall over her body and face. But when Kyle opens the door and sees her, there are no sheets of paper on her face anymore, just two sheets on her body. (00:59:00 - 00:59:55)
Continuity mistake: In one wide shot as the fire trucks drive down the road, it is apparent that Kyle's red ford, which she crashed earlier, is not there. It should be partially visible in this shot, yet isn't. (00:59:20)
Factual error: The Chicago Fire Dept fire truck is red but the real Chicago Fire Department colors are black over red. (00:59:20)
Continuity mistake: When Kyle is pursuing Andy and Chucky, she sees them climbing onto a truck which drives off. The truck is several feet away from Kyle and while it is driving it passes by a fire truck. Kyle start running after the truck and when it cuts she is right behind the truck and the fire truck is not to be seen anywhere. (01:00:00)
Continuity mistake: When Kyle manages to stop the truck, we see Andy running away with Chucky on his shoulders. It cuts to Kyle and the driver for a second before Kyle runs after Andy. In the next shot, we see Kyle running but we don't see Andy. Andy could not have run so fast that we wouldn't be able to see him anymore. (01:02:05)
Continuity mistake: When Kyle is running after Andy against the factory, we see Andy approaching a huge truck. However Andy passes the truck several times between the shots. (01:02:20)
Continuity mistake: Street goes from wet to dry, as Kyle runs toward the closing metal door at the Play Pals factory. (01:02:30)
Other mistake: No warehouse would stack their products in a labyrinthial way. They would be stacked in rows. (01:05:30)
Continuity mistake: When Kyle and Andy are climbing up the metal roller conveyer, in a wide shot when Kyle calls "Come on" to Andy, you can see Kyle is right at the top of the conveyer, but when the shot changes, Kyle isn't quite at the top yet. (01:06:30)
Continuity mistake: When Chucky is pursuing Andy up the slide we first see Chucky all they way down on the slide and Andy half way up. When it cuts Chucky is suddenly at Andy's feet. (01:07:00)
Continuity mistake: When we watch the production of the Good Guy dolls we see that the dolls get their hair stitched on while naked. However later we see dressed dolls come out of the machine with no hair on. It makes no sense that a factory would have two different production routines like that. (01:11:25)
Deliberate mistake: When Andy opens the tap to the liquid plastic it is that some of the liquid squirts on his hand. That would hurt and stain, but Andy doesn't seem to care... (01:14:35)
Continuity mistake: At the beginning of the film the facial features of the doll are sprayed on using a mask with cut out sections. There is no paint sprayed on the 'eyelashes' part of the mask, and when the mask is removed the doll has no eyelashes. Chucky, however, does have eyelashes throughout the rest of the film.
Revealing mistake: In the full-screen version, a piece of Chucky's animatronic equipment can be seen bobbing up and down with him (the bottom-left corner) as he chases Miss Kettlewell in the classroom.
Other mistake: When Andy is in the Chucky Factory at the end, the boxes momentarily knock him out, as the camera is on him, you can see that his eyes are blinking when they're shut.
Other mistake: At the beginning, we see the workers attaching Chuck's head to his body. But there's no place for the wires that would operate the dolls, seeing as how the workers believe he's just a normal doll.
