Visible crew/equipment: When Kyle is fighting Chucky, she walks backwards into Joanne's dead body and jumps. Behind her in the mirror you can see the reflection of a boom mike bob down for a couple of seconds. (00:53:05)
Visible crew/equipment: When Mattson gets in his car, you can see a crewmember wearing a yellow raincoat reflected in the car door, along with what looks like camera equipment. (00:14:50)
Visible crew/equipment: When Kyle is gardening outside, a crane is on her left. Watch carefully and a shadow moves along it. It could not be a cloud making the shadow as none of the other shadows disappear. Something or someone are making those shadows; it's either a crew member or some equipment. Kyle and Andy are supposed to be alone in the backyard. (00:26:15)
Visible crew/equipment: When in her office, Grace grabs Chucky, then Chucky stabs her twice. After he stabs her, in one shot she stumbles back, and as she does you can clearly see the head of a crewmember behind her! (00:58:40)
Visible crew/equipment: When Miss Kettlewell leaves the classroom with Andy in it, as she opens and closes the door, above the door some sort of crew equipment can be seen. It is NOT anything connected to the door, as it is not seen in previous or following shots. (00:34:25)
Visible crew/equipment: When Andy goes into the closet and he knocks the skateboard, and Chucky falls down, you can see a wire attached behind the skateboard so it doesn't fall down on the actor. (00:13:55)

Visible crew/equipment: After Andy has came back from his school, Phil and Joanne are arguing in their room about Andy and his problems. Then there is a travelling shot of the wall in direction to Andy's room, in the travelling shot there's an old black and white photograph (6th) of a woman which reflects the movie camera man and one of his hands while Phil says "For whom Joanne? For him?" (00:40:35)
Visible crew/equipment: When Andy goes upstairs after being introduced into the Simpson's home, he walks down the hallway and then backtracks to Kyle's bedroom door and opens it. As the shot pans closer in as he opens the door, the camera and its operator are shadowed on the door. (00:12:05)
Visible crew/equipment: In the very last part of the film where a new mould of a doll face is made and it grins (obviously showing there's a sequel coming), wait a few seconds after he grins and look in the background in the right corner. You can very clearly see a stage worker go right on camera. In fact, one of the frames that shows him freezes for a little while as the credits start to roll. Only in the extended TV version.
Visible crew/equipment: At the end, when Chucky looses his hand, you can see the puppeteer's hair at the bottom left corner of the screen.
Visible crew/equipment: After Andy knocks into something in the basement, in the next shot from behind, you can see a crewmember's head in the bottom-right of the screen, ducking down. (00:42:20)
Visible crew/equipment: When Chucky says "We're home!" and knocks Andy out at the toy factory, before trying to transfer his soul, watch closely. Right as the shot is about to end, you can see some of the animatronic cables, the puppeteers use to control the Chucky doll, behind him. Additionally, you can see a crew-member's hand start to reach up at the bottom left corner right before the shot cuts. (Presumably to help slow down actor Alex Vincent and the Chucky doll's fall for safety.)
Visible crew/equipment: In the scene where Andy's foster parents are talking about Chucky and the camera slides from their room to Andy's, you can see the camera's reflection in one or a few pictures on the wall.
