Revealing mistake: While cuddling in the car Donna tells her boyfriend that she isn't wearing a bra when it is obvious that she is - at one stage when she leans toward him her sweater tightens across her back and the shoulder strap and clasp of her bra is outlined.
Gourmet Night (a.k.a. Gourmet's Paradise) - S1-E5
Revealing mistake: When the Colonel first meets Basil, the same shot is used twice of the Colonel twitching.
Revealing mistake: During the fight scene at the end when Buffy is fighting the demons you see her throw one of the demons off the platform. If you watch him fall you can see him land safely on a conveniently placed mat. (00:38:10)

Revealing mistake: When Sheldon gets the sword for Penny in the game, Penny walks out and you can see the laptop isn't switched on. (00:07:55)

Revealing mistake: At the very beginning of the episode, Leonard Pearce is waiting for the cue for his character to start looking for his teeth. He's standing in the living room, between his room and Del and Rodney's room. (00:00:25)
Revealing mistake: Haley's pregnancy pad is visible when she raises her arms while lying on the table. (00:21:15)

Revealing mistake: Samuel is very much dead, laid out on the table in the canning factory employee lounge, but as Sam stands over him, talking about whether or not it was right to gank him, you can see Samuel's chest rising and falling as he breathes. (00:26:30)
Gabby's Gold - S3-E8
Revealing mistake: When Kim and Gabby swing from the bar to kick the two villains, its obvious that Gabby kicked the guy in the knees, but he falls holding his chest. Plus Kim was a good 8-12 inches from her target and he fell anyway.
Revealing mistake: (Part 4) After Omega takes off his mask and looks in the mirror, when he sees that his physical body doesn't exist he puts his mask back on and screams, but when he shouts, "If I exist only by my will, then my will is to destroy," Omega's mask pops up off his face, and we can see the actor's blackened face as he shouts the last bit of his dialogue. Revealing mistake: At the Brown Derby, when Lucy bumps into the waiter causing him to trip and spill the tray all over William Holden, we can see that the waiter's hand is taped to the bottom of the tray for the stunt to be done efficiently. (00:15:15)
Revealing mistake: Early in the episode the dead body in the barn ever so briefly flutters his eyelids.
Revealing mistake: When Sookie finds Dawn dead, in the close-up of Dawn's body, you can see the actress' pulse at the base of her neck. (00:50:55)
Chapter One: The River's Edge - S1-E1
Revealing mistake: When Hermione opens the bag of money, one stack says "Motion Picture Use Only." (00:29:45)
Revealing mistake: The headless motorcycle rider's torso and arms are not in proportion to a real motorbike rider. The torso is obviously made larger to accommodate the stunt bike rider.
Revealing mistake: When the Capri crashes into the goal net & they go to arrest the suspect you can see sky satellite dishes on the houses in front of them. Satellite TV didn't start in the UK until the 80s.
Revealing mistake: There are numerous times throughout the series where the supposed big city of Metropolis is actually just a back lot set and none of the buildings are more than a couple of storeys high. Quite often the green hills of Southern California can be seen beyond the set and between the buildings.
