Factual error: The story is set during one of Chicago's worst winters, complete with snow and ice, but the footage of the 747 Carl Kolchak is flying on shows the plane taking off from an airfield without snow or ice to be seen.

Continuity mistake: When the 747 takes off from the airport, the livery (airline colours) of the plane changes three times before it lands, including stock footage of a Boeing 747 sporting Boeing logos and the model of the plane (747).
Continuity mistake: The lit flashlight dropped on the floor by the soon-to-be beheaded victim disappears in subsequent shots.
Revealing mistake: The headless motorcycle rider's torso and arms are not in proportion to a real motorbike rider. The torso is obviously made larger to accommodate the stunt bike rider.
Revealing mistake: The pre-cut hole in the garage door is visible before and after the motorcycle busts through it.

Revealing mistake: The bulky material/block that secures the crossbow arrow shot into Miss Emily's torso can be seen under her clothing.

Continuity mistake: After Kolchak spears the moss monster in the sewer, he grabs the ladder with both hands. The next overhead shot of him shows him grabbing the ladder again.
Bad Medicine - S1-E8
Continuity mistake: A female worker in the newsroom is seen behind Miss Emily as Emily speaks to Vincenzo and Kolchak, then walks away. In the closeup shot of Miss Emily, the worker is again behind Emily with no time for the worker to return.
Visible crew/equipment: When the gust of wind blows into Helen's temple, the shadow of the giant fan creating the effect is seen.
Continuity mistake: When Kolchak enters Pops' store, Pops places his newspaper on the counter. Scene cuts and he still has the newspaper in hand.
Visible crew/equipment: A boom mike drops several times from the top into the scene of Kolchak and Pepe Torres' office.
Visible crew/equipment: When he opens the door to enter the Biology classroom, a crew member's hand is seen grabbing hold of the door as it swings open.
Revealing mistake: The footage of the traffic in front of Springfield Hospital where Helen Lynch is recuperating is recycled later in the episode.
Continuity mistake: When Kolchak looks through the window blinds at the crooks on the street, his fingers change position on the blinds as the scene cuts.
Continuity mistake: When Kolchak approaches Madelaine with the witch's bag, it is upright in his hand. The scene quickly cuts and now the bag is hanging down in his fist.
Continuity mistake: Stock footage shows the same dress mannequins moving from and then staying in their same positions, even after moving and attacking Kolchak.
Continuity mistake: Mr. Kitzmiller's secretary is about eye level with Kolchak when the camera shot is over Kolchak's left shoulder, but in the shots over the secretary's right shoulder she towers over him.
