Revealing mistake: When the typewriter is being used at the end, the keys being pressed don't correspond to the words appearing on the paper. Obvious from the start - the first letter pressed is on the middle row of the keyboard, but the first word typed is "unacceptable".
Revealing mistake: When Connor and Cameron are trying to save Derek Reese from the police truck, Cameron walks on the Jeep's hood to jump onto the police truck. During her walk, we can see that it isn't Cameron but a stunt double. She doesn't look like Cameron and is a bit taller. (00:33:05)
Revealing mistake: During "Brand New Day", watch the background as Dr. Horrible appears as a giant and stomps down the city-street. As he takes his first step, a piece of concrete from building to his right falls off and hits the ground. However, if you look closely, it's obviously a quickly-made computer effect. As the piece of concrete hits the ground, it simply vanishes, and is replaced by a rather odd-looking animated "poof" of dust. The issue is the concrete piece was much bigger than the cloud of dust, so it's clear that the piece simply disappears right as the dust-poof appears.
Revealing mistake: When Zack and Cody are tied up by the secret spy, when Cody is texting Bailey "Help Me" with his toes, you can see that the Mobile has a black screen, and is turned off. And he was just hitting random buttons with his toes.
Revealing mistake: When Nero and Gemma are talking, a woman can be seen in the background moving a paint roller up and down without actually touching anything. (00:54:45)
Iron Forged in Fire: Part One - S1-E1
Revealing mistake: After Howard’s death, Tony and Rhodey are trying to get back into Stark International. A guard is giving them a hard time and he is wearing some sort of shades and head piece. When it cuts to a close up of his face as he talks to the people inside, on the left side of his glasses near where it meets his hair, there is a texture glitch in the animation. The glass in one spot is flickering around like TV static. (00:07:10)
Revealing mistake: When Grievous falls to the ground as the Gungans throw plasma balls at him, the lance General Tarpals stuck through his torso blatantly clips through the ground. (00:15:03)

Revealing mistake: In the scene where Mitchell first visits McNair's van, Mitchell's reflection is visible in the window on the left side of the screen during the shot where he first opens the van door. The vampire lore of the show informs the viewer many, many times that vampires (such as Mitchell) and ghosts cannot be seen in reflections, pictures, or videos (their image "cannot be captured"). Revealing mistake: Near the end of the episode, when Dr. Ogden and Murdoch look closely at Lukas' dead body (right before the candle scene), the "corpse" takes a deep breath.
Revealing mistake: In practically all the episodes, where we see Darken Rahl wearing a sleeveless shirt, there are visible vaccination scars on his shoulders.
Revealing mistake: In the morgue when Mike is just about to cover the first dead body, if you look closely you will see the body's eye move. (00:04:10)

Revealing mistake: At the very beginning of the episode, as the knights are about to send Merlin to Camelot with Lancelot, the knights walk towards the camera.
