Continuity mistake: When the immortal who's trying to kill Tessa snipes at her in Duncan's car, his shot shatters the passenger-side window. When Duncan tails Tessa and her police escort to the safehouse, the passenger-side window is intact. (00:30:05 - 00:35:50)

Continuity mistake: In the episode when Penny went to work with her mom and the Magician comes in with the tiger that is acting strangely. The tiger is sitting on the floor and has just eaten its food. Penny is standing there next to the tiger, the camera goes to her mom for about two seconds, then it goes back on her and the food has returned in the bowl again. Then it's empty again.

Factual error: Jonah tells Richard that Saturn has a "rogue moon" in a retrograde orbit that travels "at four times the speed" of Saturn's other moons. The graphic on Jonah's computer shows that the moon in question is Phoebe, which does indeed orbit in the opposite direction from other Saturnian moons. And the graphic correctly shows Phoebe's orbit as being much further from the planet than the other moons. However, as a direct consequence of its greater orbital radius, Phoebe is actually among Saturn's *slowest* moons. It has a longer orbital period (or "month") and a slower orbital velocity than all the Saturnian moons with smaller orbital radii, of which there are twenty.

Episode II - The Samurai Called Jack - S1-E2
Revealing mistake: When Jack cuts the flying car's hood off, he slashes it vertically, but it is cut horizontally.

The Powerpuff Girls Best Rainy Day Adventure Ever / Just Desserts - S2-E10
Plot hole: The children decided that they hated the Powerpuff girls in one day. Then how did they have supervillain costumes and a van? The mom said they had lots of time to prepare.

Revealing mistake: When Shalimar is getting ready to leap on Gabriel, there is a quick glimpse of support wires attached to her.

Forever Red - S1-E34
Plot hole: Tommy states that he and the other Zeo Rangers destroyed the Machine Empire's leaders and ultimately destroyed the Machine Empire except for the Generals who escaped. This is not true - Lord Zedd and Rita destroyed the Machine Empire leaders at the end of series 4, and the Space Rangers destroyed them again in Countdown to Destruction in series 6. The Zeo Rangers never destroyed the Machine Empire.
Suggested correction: How would Tommy know that? He didn't watch the show or know the ins and outs of what happened in space. This is what he believed based on his experiences on Earth. Everything you described is factually correct, but Tommy did not witness this. So, to his knowledge, he destroyed the Machine Empire.

Plot hole: Bonnie is the anchor for the other side and feels when supernatural beings die. How come she didn't know Katherine wasn't dead? Did she never think it was suspicious that she never felt Katherine's body after she died?
Suggested correction: Katherine died but didn't go to the other side. So she didn't need to go through Bonnie. She went to hell to Arcadius. Bonnie is not the anchor to hell, we don't know how going to hell works.
After everyone finally realises that Elena had been possessed by her Doppelgänger Predecessor, It is actually shown that Katherine were more or less dragged away from Bonnie before she could crossover to the Otherside. So we do have some idea as to how one ends up in Hell.
Suggested correction: Katherine was a human technically when she died.
Technically, she was supernatural because she was a doppelganger, so she should've gone through Bonnie.
And even she was a traveller.

Factual error: In the show Gaius wears glasses, except eyeglasses first came into use in the late 13th century while the legend of King Arthur is around the 5th or 6th century.

Other mistake: If you look closely at the army of Rangers in the opening credits, you can see four versions of Tommy Oliver: Green Ranger, White Ranger, Zeo Ranger Red, Black Dino Ranger. Even though he had a single clone made, he can't be four variations at the same time.

Revealing mistake: The swords Jayden and Dekker use often bend when striking each other, revealing they are rubber blades and not metal.

Continuity mistake: The 4 Space Rangers (Carlos, TJ, Cassie, Ashley) morph on the overhanging gangway, then jump off to the ground below. We see them hit the ground, and in the very next shot, they're right next to the Galaxy Rangers, and Andross, who are on the other side of the courtyard. There wasn't enough time for them to move that distance.

Fortune Cookie / Dear John - S3-E8
Continuity mistake: When Filburt first reads his fortune cookie, it says "Bad luck and misfortune will infest your pathetic soul for all eternity". But later on when he is walking down the street it says "Bad luck and extreme misfortune will infest your pathetic soul for all eternity". (00:03:25 - 00:04:25)

Delenda Est - S3-E11
Revealing mistake: When Dylan and Tyr head to a bridge where supposedly the power source is located as well as Rommie, they start to cross the bridge. As the camera is facing Dylan and Tyr, the next scene two aliens teleport in. Rewind the video to facing Dylan and Tyr and play in slow motion. Watch the bottom portion of the screen and notice the top portion of one of the alien's head go into the camera as if the aliens walked into position for the teleportation.

When Two Worlds Collide - S5-E3
Continuity mistake: Gary has to feel around in mid air for the portal as the wall has just been bombed. But when he reaches through, later, his arm comes out of the wall which has somehow returned.

Factual error: One of the naked female dancers at the party has breast implants. Although some forms of plastic surgery were known to the Romans, breast augmentation was not one of them. If you want to check for yourself she appears just after the masked dwarf moves through the shot.

Continuity mistake: In Episode 5, "Live Hard, Die Young and Leave A Good Looking Corpse": As Lillie is preparing to cut Zane's throat with her fingernail, she stands with her hand raised above her head for a while. When seen from the front, her hand is balled into a fist with the index finger extended, but when seen from the back all her fingers are extended. The view alternates like this several times, and each time the hand varies between fist and outstretched fingers.

The Best Years of Your Death - S1-E3
Plot hole: In the chapel, you can see the heads of those who tried to cross Harriet (the mad matron) preserved in formaldehyde. How did she get hold of so much of it? Wouldn't her husband or the headmaster be suspicious?

The Mind of Simon Foster - S3-E23
Continuity mistake: When the pawnbroker closes his metal gate, he's facing the wire mesh. When the shot cuts, he's instantly facing away from it. (00:10:15)

Triumvirate of Terror! - S3-E8
Continuity mistake: After Joker subdues the other Amazonians, he is disguised as a female warrior and talks to Wonder Woman and her mother. There are no Amazonians knocked out in front of Joker and one is beside him with her head behind him. But in the reverse angle, that one has vanished and there is suddenly another Amazonian adjacent in front of Joker with her body in a different position. (00:11:10)
