Visible crew/equipment: When the son of the accident victims is being led through the trauma rooms to see his parents' bodies, you can see Laura Innes crouching under a table in the background directing the scene. Innes was the director of this episode and it was established that this episode took place on her character's day off. Therefore, she should not have appeared in front of the camera at all. Continuity mistake: When Jodie hits the deer her vehicle spins out. As it is spinning the headlights are not projecting any kind of visible beam. After the car stops, and she is facing the deer she hit, the headlight is now projecting a very bright beam to illuminate the deer on the road.
Continuity mistake: During the party, Stass the assassin, dressed as a waiter, hides his gun underneath a stack of white napkins. When he gets ready to pull the gun out, it is now between the napkins.
Sophia Lopez - S1-E4
Continuity mistake: When Matt talks to the "fluffer", her lollipop disappears from her mouth in between shots. She then raises her arm and puts the lollipop back in her mouth. (00:20:35)
You Can't Miss The Bear - S1-E1
Continuity mistake: When Nancy goes into her guest room with Silas and Quinne in it, the things she are holding change position in between shots. (00:28:35)
Revealing mistake: In the opening scene during the game, one of the shots of the boy is reversed. You can see that the text "Tartans" on his jersey is mirror imaged. (00:00:25)
$35K O.B.O. - S1-E18
Continuity mistake: In one shot Grissom puts a shoe on its side with a pencil. In the next shot the shoe is upright again.
Visible crew/equipment: When Nina's just been shot, and makes it to the barn to call CTU (around 7.10am), while she's speaking on the phone there's a cameraman visible on the right hand side (DVD). (00:12:40)
Revealing mistake: In the last scene of the episode, Angel kicks the guy out of the glass building. This guy's a vampire, and you can see his reflection in the glass windows. Vampires aren't supposed to have reflections.
Beware Greeks Bearing Gifts - S1-E12
Visible crew/equipment: As the people of Troy are about to be attacked by the warriors in the horse, the camera pans up to the side of the wall where Deiphobus is talking with another soldier. You can see a crew member crouching down behind them wearing a white and grey shirt. (00:30:35)
Running to Stand Still - S1-E6
Visible crew/equipment: When Susan is following Paul in the car she borrowed, we can see the reflection of a crew member on the right hand side of the window. (00:18:15)
Visible crew/equipment: When Claire runs away from Bob after he calls her "Ms. Bennet", you can see Bob's shadow and another person's shadow on the ground. Since no one else was standing next to them, it has to be a crew member, most likely a camera man based on the odd shape of the shadow.
Continuity mistake: The red Harvard sweater Andie is wearing disappears between shots.
Suggested correction: Actually, it shows her taking it off and continuing to talk to Jack.
Continuity mistake: When Izzie is looking at the patient board, there are some lines on it which have been wiped out. But in the next scene we actually see Meredith walk to the board and wipe these exact same lines off (even though they were previously rubbed out...) (00:34:05)
Visible crew/equipment: After Dr. Melfi tells Tony, "I know seniors who are inspired and inspiring," when it cuts to Christopher and Brendan waiting outside the club, the red T-mark taped to the ground is visible at Adriana's feet.
Continuity mistake: Phoebe is going into the school in one scene, but as the camera moves she vanishes while on screen.
Birthright - S3-E11
Visible crew/equipment: When Prentiss, Morgan, Rossi, and Caulfield go see Mary Wilkinson, in the wide shot just as Caulfield says, "Hello, Mary," the second camera filming from another angle is visible at the right side of the screen. Additionally, later when Chrissy confronts Charlie she's wearing one earring in her right ear, but after she shoots him that one earring is in her left ear. (00:18:45)

Continuity mistake: After Sam and Robin are done playing darts, Sam puts his darts on the pool table. The shot changes and the darts have moved.
In the Dark - S4-E22
Revealing mistake: When Tony is telling Gibbs & Ducky that Abby got a hit on the dead sailor's fingerprints, Gibbs takes the file from Tony; the camera shows the file, which appears to be a legitimate profile sheet, but in the comments section, is the script that Tony just read aloud. (00:07:36)
