Revealing mistake: It's pretty obvious that Scott Glenn, the actor playing Stick, just has his hand wrapped up to look like it has been cut off. Both his hands are at equal length when his arms are down by his sides.
Revealing mistake: While Holmes and Watson are investigating the crime scene, there is a shot of them that shows the dead man's hand. His hand is visibly shaking, revealing the actor is struggling to maintain the position of his hand.
The Killer - S1-E1

Revealing mistake: After DC Louise Gardiner dies, DCI Jim Keats closes her eyelids. She swallows as he moves his hand across her face.
Deathfist 5: Major Crimes Unit - S2-E15
Revealing mistake: There's a stunt featuring two-way too obvious body doubles for Kelly Hu and Gretchen Egolf rolling over a table; not only the stunt itself is rather underwhelming and blatant, but also the objects on the table disappear just so the two can perform it.
Saving Face - S3-E3
Revealing mistake: After they drop the casket and Provenza says 'guess you can take it with you!', the close up of the dead girl shows her close her mouth.
Revealing mistake: Every phone call in the show, the phones never show the correct screen. Either it is glitching behind their ears or it's the home screen.
The Countess - S1-E4
Revealing mistake: When the car falls off a cliff at the end, it's an entirely different looking model with different headlights (square vs round), and empty (at least nobody in the passenger seat for sure). (00:46:30)
Revealing mistake: The doctors remove a bullet from Elliot's abdomen and place it in a metal medical dish. The bullet appears to be a 9mm or a 9mm short however it has no striations. When a bullet is fired it goes through the barrel picking up the land and grooves of the bore. The bullet should have spirals, this one looks brand-new, like it was just purchased to reload ammunition. (00:48:30 - 00:49:00)
Revealing mistake: In the scene where the wife comes in to the morgue to view her dead husband, who she helped to euthanize, the dead man is breathing throughout the whole scene. You can see the sheet rising and falling at his stomach. (00:22:15 - 00:24:05)
The Affair - S1-E4
Revealing mistake: The attempt on Rodriguez's life is quite phony looking; leaving aside the trope of a person who wants to avoid being murdered by a car by trying to outrun it instead of getting out of the way (the first semi-POV of the car does a terrible job at that, since the car looks much closer to The Victim and slower than it should be, and wide room for Rodriguez to escape sideways), the impact does not happen and the stuntman jumps into a pile of garbage without anything remotely close to connecting. Revealing mistake: When everyone is fighting with the water buckets, Don Ramón (Raymond) throws water at Quico. Doña Florinda (Mrs. Worthmore) chases Don Ramón inside the neighborhood. When Quico was about to throw water at Chavo, inside the door, the shadow of Don Ramón waiting for running so Quico can throw water at his mother. Plus, when Don Ramón and Doña Florinda runs outside the neighborhood, Doña Florinda is already covering herself in the last second before getting wet.
Insane in the Germ Brain - S4-E11
Revealing mistake: Near the end of the episode, a zombie hating teenager barges into the church to assassinate brother love. He ends up shooting a choir member instead. Although the gun goes off, blood splatters as if it were a head shot, and the man collapses, there is no point of entry. They forgot to CGI the headshot.
Revealing mistake: When Kojak drives to the store where the hostage situation takes place, the side mirror of the interior view does not match the car he arrives in. The footage is also noticeably sped up at the last turn, when he passes by a Delicatessen window and a Rent-a-car sign. (00:09:30)
Revealing mistake: About 20 minutes in, the cops have a newspaper with the headline about the two guys they killed. They turn to page two which has a story about the next guy they are planning to kill. If you look at the picture, it is glued onto the story page. The left side is lifted. The actor must have been hired after the printing.
Revealing mistake: The FBI woman picks up the phone to speak to her boss but the phone is off. (00:22:38)
