Revealing mistake: When the target's iPhone rings and Christopher Chance answers it, he swipes his finger across the bottom of the screen, which is the regular gesture to answer a call on the iPhone. Trouble is that we see the screen at the time, and it's not showing an incoming call, just some sort of list - he swipes his finger over nothing of any significance at all.
Revealing mistake: In the scene where the wife comes in to the morgue to view her dead husband, who she helped to euthanize, the dead man is breathing throughout the whole scene. You can see the sheet rising and falling at his stomach. (00:22:15 - 00:24:05)
Sweet Johnny - S4-E5
Revealing mistake: The air mattress embedded in the ground that cushions their fall, and the curly hair and face of the stuntman for Earl are quite obvious when Earl and Sweet Johnny hit the ground.
Revealing mistake: About 20 minutes in, the cops have a newspaper with the headline about the two guys they killed. They turn to page two which has a story about the next guy they are planning to kill. If you look at the picture, it is glued onto the story page. The left side is lifted. The actor must have been hired after the printing.
Revealing mistake: In the arcade the killer talks to his helper while playing a pinball machine. He plays very agitated and strongly. But if you look closer at the below end of the playfield you see that there is no ball and the flippers aren't moving. (00:23:50 - 00:24:48)

Revealing mistake: WPC Softly leans on a model building in Rammel's office, causing it to collapse by stages. The wire that is being pulled to make the model collapse can be seen to the right of the model. (00:21:00)

Revealing mistake: On the RV of the guy being accused of molesting the girls, someone has written "PERVERT" in red spray paint. At a point you see Lightman next to a car, and the RV in the background, and the text is mirrored reading: "TREVREP", showing that during editing, this shot was flipped. (00:15:30)
Revealing mistake: The dead victim is being examined in the morgue, right as the mortician says "the kid put up a fight" the dead victim swallows as she strokes his hair and says the word "fight." (00:09:35)

Revealing mistake: There's a stunt featuring two-way too obvious body doubles for Kelly Hu and Gretchen Egolf rolling over a table; not only the stunt itself is rather underwhelming and blatant, but also the objects on the table disappear just so the two can perform it.
Withdrawal - S2-E1
Revealing mistake: At 46:24, Peter and Mozzie are sat on a bench. Peter's paper has the exact same wording on back pages as well as the front page. (00:46:24)
Saving Face - S3-E3
Revealing mistake: After they drop the casket and Provenza says 'guess you can take it with you!', the close up of the dead girl shows her close her mouth.
The Delivery - S7-E6
Revealing mistake: Eddie's bar has just burned down, and everything is ruined. However, the glasses above the bar are pristine and clean. At the very least, they'd be blackened and smeared by the smoke and soot, even if the heat of the fire hadn't destroyed them. (00:10:28)
Revealing mistake: Not only is the stunt double's face visible in several shots during the apartment fight scene, the man is also noticeably taller and heavier than Robert Wagner, and is wearing a really bad wig.
Revealing mistake: As the RCV enters the gravel field with the bomb, you can see a person in shorts walking casually in the background, despite the area being sealed off.
Revealing mistake: In the forensics lab Carol Wilson is saying to others that the rat caught on the ship is healthy. In the ensuing conversation the respirators they are wearing are so ill fitting as to be pointless - Carol's mask does not even touch her nose. (00:26:00 - 00:27:00)

Revealing mistake: The exterior shot of the house of the rich Miss Elias preceding Dr. Ruben Seelman's phone call, is a zoom on a static photography with a visible line on it. It is reused later in the episode (after Jim pulverizes his own picture in the phone book), with the exact same telling line. (00:30:20)
Revealing mistake: When Kim is leaving Gatwood Oil and realises her car is stuck in the dirt, the car is still in park. No wonder it's not going anywhere.
Insane in the Germ Brain - S4-E11
Revealing mistake: Near the end of the episode, a zombie hating teenager barges into the church to assassinate brother love. He ends up shooting a choir member instead. Although the gun goes off, blood splatters as if it were a head shot, and the man collapses, there is no point of entry. They forgot to CGI the headshot.
