Iron Forged in Fire: Part One - S1-E1
Continuity mistake: After Tony says he is going to use the Iron Man armor to spy on Stane, he and Rhodey goof off some and wrestle. In the first shot of this, you see Rhodey’s arm come on camera and grab Tony’s, as Tony moves his hand up towards his own face. When he does, his arm and hand cast a shadow on his neck and chest. The shot cuts to a side view and suddenly the shadow has completely vanished. (00:15:00)
The Alexander the Greater Affair (1) - S2-E1
Character mistake: When Alexander organizes the human chess game in his courtyard, the "pieces" stand the wrong way on the board, with a dark square in the lower right hand corner. That corner is always a light square. (00:22:50)
Fine Feathered Finks (1) - S1-E3
Continuity mistake: When Batman starts the Bat-Turn, the parachutes are shown deploying on a 6-lane major roadway (3 lanes in each direction) in a very open area, but after completing the 180-degree turn, the car is shown driving away from the crumpled chutes on a much narrower side street among tall buildings. Triumvirate of Terror! - S3-E8
Continuity mistake: When Joker zaps Wonder Woman and her mother, they are both standing in front of their thrones and fall forwards. But when Wonder Woman stands back up, her mother, still unconscious, is suddenly off to the side and behind the thrones. (00:11:35)
Wonder Woman vs Gargantua - S1-E7
Visible crew/equipment: When the truck carrying the caged gorilla pulls into the alleyway, just as Hans and Carl exit the truck and walk away, the blue-sleeved arm of the cameraman/crew member is visible as it moves upward, at the right side of the screen.
Rising (1) - S1-E1
Continuity mistake: When they first arrive on Teyla's planet, the DHD is close to the gate. When the wraith attack, the DHD is much further from the gate.
Deliberate mistake: During the civilian sheriff's pursuit of the Navy car, the sheriff is occasionally visible in the passenger seat of his patrol car instead of the driver's seat.
Continuity mistake: When Tommy is escaping into the mountains to try and help, a small bit of his jacket tears off onto a tree branch. MacGyver later sees it, but the branch that the torn piece is on looks entirely different. (00:21:10 - 00:22:10)
Continuity mistake: When Kellerman orders Hale to give him his weapon, Kellerman pulls out his own weapon with his left hand. In the next shot, his gun is in his right hand, and he then transfers it back to his left. (00:36:45)
Communication Breakdown - S5-E19
Character mistake: When Mac, Stella, and Danny are trying to determine where the fired bullet entered the train car, none of them are wearing rubber gloves. But even though they're not handling any evidence, they're still processing a crime scene. (00:05:00)
Revealing mistake: When Syd's Dad rescues her in the car park, he starts shooting at the other car. After firing one shot the slide of his gun locks back (indicating it's out of bullets), but when the angle changes he carries on firing with no problems. (00:43:20)
The Odyssey - S1-E14
Continuity mistake: When Wilson jams the pistol in Oliver's face, Wilson is holding the gun in his right hand. When the shot changes, it flips - he is suddenly holding it in his left hand, and his previous un-scarred left eye now has some scrapes around it.
Continuity mistake: The package Nigel holds in his arms near the beginning of the episode completely changes between shots.
Dear Diary - S4-E20
Continuity mistake: While it's forgiven that the Generals usually have subtle changes through episodes and the series, in this episode we see a General with extremely inconsistent graphics when Bo and Luke deliver the counterfeit money to Winston. The 0 is off and the bottom of the 1 is extremely long.
Continuity mistake: When the TV star is killed he is wearing a white t-shirt with black lettering. As he is dragged to and placed in the trunk of the car it is still visible. When the CSIs move him from the trunk onto the gurney, there is no lettering on the t-shirt.
Continuity mistake: During the car chase, the NSA's black SUV rams into Chuck's little car. The front license plate breaks off and is hanging on by one corner. When it's backing down the stairs, the plate has somehow reattached itself perfectly to the bumper. (00:29:20)
Continuity mistake: When Starfire asks what is the Maximum 7, her, Robin, Raven, and Beast Boy are standing together looking at Cyborg in a wide shot. Raven is slightly behind Beast Boy but to his right, and her body is facing to her left. But then it cuts to a close shot from in front of the Titans and suddenly Raven is on Beast Boy's left and right beside him, with her body facing her right. (00:03:50)
Continuity mistake: When the door of Pod 1 opens after Jeff realises there is a saboteur on board, we see three vehicles in the pod and beside the Monobrake there is a red unit which has a windshield on it. However, when we see Chip appearing in the next shot, the windshield has gone.
Visible crew/equipment: After Hutch finds the ticking box with the clown and Starsky's badge inside, when it cuts to Hutch and Dobey walking toward the squad room, the cameraman and camera are reflected on the door's glass window. (00:17:20)
Continuity mistake: While trying to restart the engines after the ship is hit by the nuke's EMP, Chandler is checking the fuses while Chung is going through his checklist. There are only five visible fuses in the six available slots in the panel that Chandler is checking. After he gives the order for Chung to "give it a kick-start," the slot which previously had no fuse now does, only for it to shoot out from the start attempt.
Suggested correction: Theoretically correct, but it can be argued that the only evidence present is the bullet (which has DNA and prints burnt off when fired) and the body (which can only be handled by the MEs) gloves are not required.

What utter rubbish. No CSI would enter a crime scene without first putting on latex gloves - it is a carved-in-stone rule.
Exactly. Since Mac, Stella and Danny were processing a crime scene, they would need to wear gloves because without them, they would have contaminated a crime scene by not only getting their fingerprints over everything but, if one of them got a cut or scrape, their DNA would also be thrown into the mix and any defense attorney can call them out on their carelessness and have any evidence thrown out.
