Other mistake: The same dungeon set is used several times throughout the series, just furnished differently.
Revealing mistake: Any time a laser is fired in the show, there are no scorch marks on the walls where they hit.
Continuity mistake: When Max is phoning the Chief to let him know the mission's been accomplished, he takes the oars out of the oarlocks before opening his shoephone. The oars change from being out of the oarlocks in the closeups to still being in the water in the long shots for the rest of the episode. (00:23:50 - 00:24:30)
Continuity mistake: When the owner of the shop is vaporized, at first his hands are both in front of him. The shot changes and now one arm is instantly thrown back behind him.
Continuity mistake: When Max is attacking the boat there is a shot from inside the cabin. Watch the door. Before Max shoots, there are a series of holes in the shade covering the door (they look like bullet holes). Max begins firing and there is another shot from inside and bullet holes begin appearing in the walls. The holes in the shade however, are gone.
Audio problem: Max is firing a weapon on the garbage scow when his gun suddenly stops. He exclaims "It's jammed!", but his lips don't move. Also, what is he firing at? There's nobody there.
School Days - S1-E3
Continuity mistake: When Max is walking around the school grounds he walks up to man breaking a piece of wood. The man breaks a piece of wood and then bends down to pick up a new piece, leaving one on the ground. The shot changes and Max is standing there talking and the man breaks the piece of wood and bends down to pick up another one (which should be the last piece). However there is another piece left on the ground.

Revealing mistake: When Max is being attacked with throwing knives, one is thrown into the closet door. Watch closely; the wire used to guide the knife is visible.
Washington 4, Indians 3 - S1-E6
Other mistake: When Max tricks the Indian's daughter, he is supposed to say, "Now close your eyes and open your mouth". Instead he says, "Close your mouth and open your eyes". The Indian woman still reacts with closed eyes and open mouth.
Continuity mistake: At the end when Max is receiving his award, he accidentally immobilises everyone in the room. The chief changes position in every shot, despite being immobilised.
Visible crew/equipment: When Max and 99 have reached the mansion, they both get out of the car to look in the trunk. As they start walking, some lightning occurs. If you look up at the top, you can see where the fake sky ends and the visible back of the stage begins.
Continuity mistake: When Max challenges the Shark to a game of pool, Max's hand holding his cigarette jumps to the tip of his pool cue, and a chalking cube appears in his hand from nowhere. (00:19:00)

Continuity mistake: When the chief stops Maxwell from beating the cab driver, he holds back Maxwell's arm by the very prominent cuff. After the following cut he holds it by the sleeve (and no sign of the cuff). (00:05:50)
Character mistake: Maxwell translates his famous line "And loving it' into the German 'Und damit gefreut'. Agent 99 who claims to speak German fluently confirms it. This translation is both far away from the meaning and doesn't make any sense at all. The right translation is 'Und liebe es'. (00:09:50)

Revealing mistake: When the fake Chief calls Max over to the car, the driver is meant to be a double of Smart, but the driver is not Don Adams.
