Continuity mistake: When Sheldon and Leonard are in the kitchen discussing Leonard's upcoming night out with Mrs. Latham, there's a plastic bottle of orange juice on the table that keeps disappearing and reappearing as the camera switches between Sheldon and Leonard.
Suggested correction: The bottle is there every time, a few times because of the camera angle it is hidden by Sheldon.
The bottle of OJ is right next to a container of Coffee Mate, both items always visible when the bottle of OJ is present. When the OJ is not present, only the Coffee Mate is there, near enough the center of the table to not possibly be blocked out by either Sheldon nor Leonard.
No it is not right next to the coffee Mate, there is some distance with a phone in between. You can sometimes see the bottle behind Sheldon when he moves.

Continuity mistake: When Penny is pouring water into the glasses on the table after first meeting Raj's sister. The level of water in the pitcher changes between shots.

Continuity mistake: Near the end of the episode, in the canteen scene, Howard shows the card trick to Sheldon again. The card at the bottom of the deck is the 2 of hearts as he takes them from the box. Sheldon then takes this card from near the top of the deck when Howard puts them on the table. The deck is not shuffled between times.

Continuity mistake: It is well-known that Leonard's glasses do not have lenses to avoid catching reflection by the lights. In this episode, Priya takes Leonard's glasses off, and they have lenses.

Continuity mistake: When the girls take Sheldon out dancing, he mentions that he considers himself a proficient dancer due to his Cotillion training when he was a boy. But in S1E7 The Dumpling Paradox, when Leonard suggests that they could have gone dancing with the girls, Sheldon said that nobody in the group - including him - could dance.

Continuity mistake: Leonard refuses Penny's offer of a glass of wine because the sulphates in wine give him migraines. This makes no sense - we have seen him drinking wine in too many episodes to count without him once complaining of an adverse reaction.

The Shiny Trinket Maneuver - S5-E12
Continuity mistake: Howard, Leonard and Raj play Jenga in a strange sort of way. Most notable is the playing order, 1. Howard, 2. Raj, 3. Raj, 4. Leonard, 5. Howard. When it's Leonard's turn he doesn't move any bricks, only pokes the tower and it's then Howard's turn. When its Raj's first turn he puts a brick on the top of the tower. When it's his turn again, the brick is gone and he puts a new one in its place.
The Rothman Disintegration - S5-E17
Continuity mistake: After Amy returns to the apartment for the Grease DVD and discovers Penny doesn't like the painting, she then exits the apartment with the painting and the DVD with her. When Amy says to Penny she doesn't need her pity, the DVD is placed above the hanging string of the painting. In the next shot, as she walks into the hallway, the DVD is suddenly almost half below the string.
The Werewolf Transformation - S5-E18
Continuity mistake: When Penny is giving Sheldon his haircut, she accidentally shaves the back of his head. The clippers are going from the bottom of Sheldon's hair, up to the top of his head. When Sheldon goes to leave the room, it can be seen only four or five inches from the back of his head have been shaved off.

Continuity mistake: Sheldon opens his transporter box and takes out Mr. Spock, while making a big deal about it remaining mint in box, unopened. Later, he makes a big show of using an exacto knife to gently open the box with Spock in it.
