Welcome to Santa Rosa County - S6-E10
Visible crew/equipment: After Roy and John eat lunch with the sheriff and decide to head to the lake after lunch, when they both get into the squad car, we can see the reflection of the cameraman, who's wearing jeans and a dark shirt, in the wing mirror, as he walks closer to the car and bends down. We can also see the legs of another crew member standing behind him.
Welcome to Santa Rosa County - S6-E10
Revealing mistake: After rescuing the burn victim from The Boat's explosion, Johnny and Roy are at the back of the camper with the victim, while the victim's wife drives the camper to the clinic, but when they arrive at the clinic the camper hasn't even stopped yet, and we can see the woman's legs at the side of the camper, waiting for her cue to come rushing to the camper's rear door when her husband is brought out.
Visible crew/equipment: At the scene of the plane that hit the building, after the explosion, when Foam 10 arrives there's a closeup of the firemen using the foam tubing, and behind the doorway one of the 'firemen' kicks the foam tubing, but notice that he's not really a fireman, he's actually a crew member who's wearing a turnout coat, though he's also wearing jeans and work boots.
Visible crew/equipment: At the start, after the boy ties a bowline knot in the rope attached to the van, when the mom gets into the van with her daughter, the reflections of two crew members, the cameraman and the top of the camera are all visible in the vent window, in two shots.
Bottom Line - S6-E17
Revealing mistake: While the guys are eating chilli at the table, when Station 51 is dispatched to 'man down', notice that when they're at the scene, both paramedics are wearing their jackets and Johnny is wearing his helmet. We never see Johnny's face in any of the wide shots, because the entire time it's the double with Roy. We only see Johnny alone in a few closeups, pickup shots which were filmed later, and we can see the style of the houses in the closeups do not resemble the houses in the other shots. The real Johnny is back, once they reach the hospital.
Revealing mistake: When Station 51 is dispatched to the gas explosion at the cafeteria, during the actual rescue the four firemen we see in closeup in the cafeteria are supposed to be Roy, Johnny, Chet, and Marco, but it's really odd that all are the doubles, very obvious in the closeups. There's little dialogue during this scene, which was dubbed later in ADR.
Revealing mistake: At the movie studio, when the actor who plays Dr. Ned is loaded onto the ambulance stretcher, their set's 'fake' window area is the same as Rampart's 'real' hallway later in the episode. We can see this when Brackett, Early and Dix walk down the hallway to kick Dr. Ned's film crew out of his room. Pretty funny to see the same chairs, wall, window with painted exterior backdrop and color stripes on the floor. This area of Rampart can also be seen in the previous episode 'Family Ties', when the clock radio shorts out causing a fire in the patient's room.
Visible crew/equipment: After Station 51 is dispatched to the back hoe that flipped over trapping the driver, when Roy gets out of Squad 51 upon arrival, the reflections of a crew member and part of the camera are visible on the squad's door.
Continuity mistake: In the bookmaker's apartment, while Roy. John, Marco and Chet are working on the bookie who's in full cardiac arrest, Vince walks in and speaks to Captain Stanley. Then Vince, who's wearing a watch and ring on his right arm/hand, reaches for all the ringing telephones, and one by one he removes the receivers from the bases, but the right arm/hand in the closeup has no ring or watch.
Visible crew/equipment: While the two victims of The Boat wreck are on the ground down below, just as the Coast Guard chopper begins to lower Roy on the line, the cameraman/crew member wearing a grey T-shirt is visible down below on the ground, just past Roy's left shoulder.
Visible crew/equipment: During the rescue of The Boat wreck off Catalina, while John's waiting on the ground with one victim, when Roy and the other victim are in the basket being lifted onto the Coast Guard chopper, the shirtless cameraman/crew member wearing a red cap and shorts is visible down below, under the chopper.

Continuity mistake: When Roy and Johnny cross the road to talk to the sheriff about the injured rock climbers, Roy does not have gloves in the back pocket of his jeans or anywhere else, nor does he take gloves from the sheriff's trunk with the other climbing gear, before he and Johnny begin their ascent. However, when he reaches the boys, Roy has a pair of thick work gloves sticking out of his back pocket.
Visible crew/equipment: When Johnny and Roy walk into the staff lounge, to join Morton, Brice, and the other paramedics for the tape review meeting, the two actors’ T-marks are visible under the table, where he and Roy are standing.
Visible crew/equipment: When Johnny is reading out loud from a transcript, while he and Roy head to the tape review meeting in the staff lounge, just after they turn the corner, the three tape marks are visible on the floor. One is for Johnny near the door, and two are for Roy - the first mark for where he originally stands and the second one for when he walks back over to Johnny.
Visible crew/equipment: When Roy, Johnny, and Morton leave the staff lounge talking about Brice, they stop at the nurses’ station, and when Johnny and Roy walk away, the two actors’ tape marks are visible where Morton is still standing and where Roy had just been standing. Additionally, the outlines of the lavalier mic transmitters are visible on the left sides of Roy's and Johnny's shirts, particularly when they stop at the fountain.
Visible crew/equipment: When the woman runs into the ER screaming that her son accidentally shot himself, as she and Brackett race to the door, the actor's black tape mark is visible on the floor behind her, where she had been standing.
Visible crew/equipment: When Brice and his partner see Brackett and he starts talking to Brackett about the regulations regarding reporters, as they walk down the hallway the actor's tape mark is visible on the floor behind them, where Bracket had been standing.
Loose Ends - S6-E12
Continuity mistake: When DeSoto unbuttons the police officer's shirt, one of the clear, round, self-adhesive pads is already on his chest, before DeSoto places any of the ECG electrodes.
Visible crew/equipment: When Squad 51 is dispatched to 'man unconscious possible heart attack', while en route, two shots after Roy says "It's gonna be a long run," the reflection of the squad is visible on windows they drive past, and we can see the reflection of a crew member who's standing on the back of the moving squad. Slow motion is not necessary to see this. However, if you're into slo-mo you can catch it again later, when John and Roy head to the traffic accident victim.
Visible crew/equipment: When Roy and Johnny respond to The Boat wreck with injuries off Catalina, they pull up to the Coast Guard Air Station, and when Roy joins Johnny on the other side of Squad 51, the moving reflections of two crew members are visible on the back of the squad.
Answer: It is standard practice to wet down driveways so that they stand out in a long shot.
