Continuity mistake: When West ties his horse to column, there is a boarded up entrance to the left with one wooden plank at a 45 degree angle in the middle. When West returns to his horse to leave, the plank is missing, and West rides off without having to untie his horse. (00:03:05 - 00:05:30)
The Night of the Deadly Blossom - S2-E25
Continuity mistake: When Barclay hangs the bone hatchet back in its place, the outline on the wall supposedly left by the item differs significantly from the artifact itself. (Most noticeable is the upper end: the outline has one long curled tip, but the actual hatchet has two short prongs there.) The outline was apparently drawn without the prop, or from a different prop altogether. (00:12:05)

Continuity mistake: In the final scene of this episode, the train is shown at twilight. When Gordon leaves the private box car, it is pitch black outside. The last shot of the episode shows the train in twilight again. (00:47:10 - 00:47:45)

Continuity mistake: The bird flies and lands on two books. To the right of the books is a red glass bottle with a frame document behind it. The next shot of bird shows it atop the red glass bottle which is now to the left of the books, away from the framed document. (00:01:10 - 00:02:45)

Continuity mistake: After a bomb explodes near West, when he stands up, there is a rip midway on the seam of the seat of his pants. In the subsequent fight scene, the tear is no longer visible. (00:28:25)

Continuity mistake: West's coat collar alternates from turned up to turned down several times as the shots change. This continues to occur throughout the episode. (00:17:30)
The Night of the Wolf - S2-E27
Continuity mistake: The boot heel West removes to obtain his hidden explosive somehow reattaches itself by the time he leaps into the mining car. (He doesn't stop to put it back - he's running for his life from a pack of hungry wolves.) His boots also change from black to brown by the time he makes it outside and jumps on his trusty horse. (00:32:40)
