Continuity mistake: Season 2, Episode 10 "Rendezvous". When Geary hits Bellic then backpack with money falls. In the next shot, before Geary hits Bellic second time we see that the backpack is standing.

Visible crew/equipment: Just as Alex works out of the numbers, the shot changes to Michael. In the closeup of his face, a white reflector screen is visible in his sunglasses. (00:20:30)
Continuity mistake: When Sarah is being tortured at the end, her hair is constantly changing position. (00:40:00)
Factual error: Linc arrives at the 'Bolshoi Booze' location at 17:45. It is an 8 hour drive from Trinidad Colorado to that location, which is right on the Texas - New Mexico border. That means that they would have had to have left at 09:45 that morning. But they don't appear to have stayed a night at Linc's father's place - and in none of the other plot lines has it been night. They couldn't have covered that ground in the time span of the plot.
Continuity mistake: When Sara is being tortured in the bathtub Paul Kellerman receives a call. When we see a brief clip of Sara, her hair is a lot drier than it was a second earlier.

Continuity mistake: When Detective Slattery gets the envelope with the receipt in it she leaves the top open and walks to her desk. Camera changes and now the envelope is closed and held differently. (00:26:20)

Plot hole: The Kansas police arrest Bellick for, and he is subsequently convicted of, Geary's murder based in part on a threatening voice mail he left on Geary's phone. But The Message's time and date would match the records of Bellick's admittance into the hospital, thus proving him innocent of the crime.
Suggested correction: Not necessarily, The Kansas police would have to do an autopsy to determine cause of death and time of death. T-bag had killed Geary long before Bellick left the voice mail as shortly after, Roy's body arrives at the hospital. So the prosecutor could easily say the voicemail was left after Roy died to try and cover his (Bellick's) tracks.

Continuity mistake: Belleck gets his bail denied, and he raises his hand to his face. Camera cuts to the front to see him perform the same action again. (00:14:39)
Visible crew/equipment: When Kellerman rings Mahone, the camera rig is reflected in Mahone's glasses. (00:25:24)
Continuity mistake: When the van comes to a stop in the tunnel, it is out of alignment with the rest of the convoy. Camera changes and the van has now shifted sideways, back into alignment. (00:26:25)
Continuity mistake: We see T-Bag standing leaning against a post - he has a pocket watch in his good hand and his bad hand is lowered. Camera cuts and now his bad hand is raised and his good hand is open palmed. (00:29:28)
