Continuity mistake: In season 1 Lincoln says his mother died of liver cancer. In season 4 they said she died from a brain tumour.
Other mistake: Michael goes to Fox River in April. Maricruz gets pregnant in the same month. In mid-June, when Michael get arrested in Panama, she's still pregnant. That's 2 months. He was in there for about 2 weeks, due to his deal with the company. Therefore, he would have escaped Sona at the end of June or the start of July. The start of Season 4 is set 3 weeks after the escape from Sona, so Maricruz should be 3 months pregnant, but gives birth at the start of season 4, meaning that the baby is 6 months early but looks very healthy.
Continuity mistake: At the car dealer where Don Self speaks to the seller, the seller says "How did you find me?" and they are inside the car dealership. Don responds with "I've never lost you". Suddenly when Don answers they are outside the building, like they teleported. (00:13:25)
Continuity mistake: After Sucre and the driver of Stuart's car wreck, another car pulls up behind and starts honking. Cut to a shot from behind the cars and there is a whole line of cars waiting. They weren't there in the opposite angle.
Audio problem: When Gretchen kills the guy in the prison he lets out a very obvious fake scream similar to a Wilhelm scream. Also, he is being choked around the neck, how is he able to make the scream? Take note of when you see this scene in the next episode's review. The guy doesn't make the scream in that version.
Visible crew/equipment: In the scene when Mahone is trying to escape from the restricted area, Sarah looks into the window of the door to see him. The camera moves in as well and you can see it reflected in the glass.
Continuity mistake: Episode 4x7, "Five the Hard Way": when Gretchen calls Agent Self at the end of the episode, Self's phone shifts from being in his right hand and ear to his left hand and ear.
Continuity mistake: When Sara comes to visit Gretchen is a little mistake. When Sara makes a little cut in Gretchen's throat, the wound looks different sometimes, and at one point completely vanishes.
Visible crew/equipment: There is a scene when the camera is moving fast around the outside of T-bag's office near the beginning. In one shot the camera person's back or leg is reflected in the glass.

Audio problem: When Mahone is speaking to Baker, just after Michael goes under the X-Ray, Baker says ' My goal is to build one city that entirely self sustains'. After this, Baker's words do not match the actor's mouth, from the shot behind him; 'Depending on no outside resources'. (00:19:45)

Revealing mistake: Sucre and Linc are driving to the pier and there is a shot looking at their SUV. In the background you can see a cop directing traffic away from the shot. It's fairly obvious.
Revealing mistake: In the scene where Sarah crashes into the agent's buggy, you see they have been replaced with dummies. When the car first hits, you see one of the dummies fall out, but they are both seated in the next shot. Also you see that it is not Sarah driving but a man. (00:36:50)
Other mistake: The psychologist shows Michael a photo of his family with his Mom, Dad, Michael and Lincoln but Michael never actually met his father when he was young.
Revealing mistake: When Sara crashes into the ATV to save Michael, you can see that the people on the ATV are switched out with crash-dummies.
Character mistake: While looking at a doctored photo of Lincoln and his mother, Lincoln mentions that Michael was born in 1976, but at the end of "Killing Your Number", it reads on Michael's grave stone that he was born in 1974.
Answer: Don Self actually worked with Whistler before he died. And Whistler with the bird book actually knew where Scylla was.
