Continuity mistake: When Abby tells McGee that the suspect has a gun, there is an overhead shot of the top of the building and the brown car is at least 4 spaces away from it. When Abby is watching the report on TV and says "damn you Gibbs", the car is next to the building. (00:04:00 - 00:05:00)
Revealing mistake: At the start, when the brain is being passed around you can see a fixed tube connecting to the student's arm to presumably pump the mercury into his hand. (00:01:20)

Visible crew/equipment: When Ducky comes in explaining how the victim has one more thing to say, a red tape marker is visible on the floor. (00:37:05)

Visible crew/equipment: Just as the blonde asks if she's needed to ID another photo, there is an overhead shot that includes the camera trolley wheel. (00:35:20)

Visible crew/equipment: When Tony is pacing after they find out Kamal's identity, there is a green tape marker visible on the floor. (00:32:00)
Factual error: During the slow motion sequence in DC, landmarks from Los Angeles are in the background. (00:11:45)
Continuity mistake: McGee, Gibbs and Ziva all agree it is Saturday morning. However when the director looks at her phone, it says it's Thursday. (00:06:40)
Audio problem: When McGee is saying that he was raised a gentleman, his mouth doesn't move. (00:04:30)
Continuity mistake: This episode takes place on the same day as the previous episode, however, Jenny's hair is now much longer. (00:02:35)

Continuity mistake: The pill bottle the agents recovered from the house has the Doctor's name "Fleming" to search for. But when McGee searches the NCIS system, he spells the name "Flemming" and gets his results. As well, the scrip sheets for Dr. Flemming archived on the computer, contain several identification numbers - DEA (AJ0000000000), Batch (00000001) & License (C00000.) - not real numbers - so the prop master didn't even try to fake any of them. (00:15:20)
Revealing mistake: Part 1 - As Tony eats his donut, and spills the jelly on his shirt, it's clear that (the actor) is moving his body to make that 'accident' happen as scripted. As the jelly begins to leak out, he doesn't move; because it doesn't fall as expected, he jerks suddenly, to make the jelly fall on his shirt. He stops moving, 'shocked' at this, holding the donut with both hands. Part 2 - The camera angle changes to 'behind' him, and he is only holding the donut with one hand. Part 3 - He then grabs at a napkin (just out of frame) to clean it off; the camera cuts to Ziva briefly, then back to Tony, who does not have any napkin. (00:24:55)
Answer: I hadn't seen the backdoor pilots yet but while it wasn't explicitly stated, at least onscreen, I heard somewhere Vivian got either fired from NCIS due to jeopardizing a case, got resigned from the agency, or transferred to the FBI.
