Revealing mistake: Batman drives out of the batcave alone. The first bit of stock footage of the car leaving the cave does indeed show him alone in the batmobile. But the very next stock shot of the car speeding down the dirt road shows us Robin sitting in the passenger seat. (00:05:25)

Visible crew/equipment: After Albert answers the Batphone it cuts to Bruce and Dick sitting at the table. We can see a chalk T-mark on the floor near Dick, and a white tape mark on the floor near Bruce. Then, when Harriet walks in another white tape mark becomes visible on the floor by her feet. True or False-Face (1) - S1-E17
Other mistake: When False Face lifts the wig off Blaze to reveal the crown that is on top of her head, you can see it is not there until the next shot of her.
Deliberate mistake: When Batman and Robin get free from the train tracks in the nick of time, the shot of the couple on the passing train is flipped; note things are backwards. (00:04:30)
Continuity mistake: When False Face escapes from the bank he has Blaze with him in the Town Panel truck, and she's handcuffed to the overhead pipe. Blaze is wearing a neutral color nail polish, but in the closeups of her cuffed hand her nail polish is salmon colored. (00:14:30)
Other mistake: While Batman and Robin are pursuing False Face, they stop the Batmobile beside a fence (across from "Bioscope"), and a sign that states "No Trespassing, Desilu Studios" (the show's studio location) is mounted on that fence. (00:15:00)
Visible crew/equipment: At the end Harriet surprises Bruce and Dick with a rehabilitated criminal, and when Blaze walks in we can see the shadow of the moving boom mic on the left side of the screen. (00:23:05)

Continuity mistake: Catwoman dangles an unconscious Robin face-down on a tilting board over her tiger pit. He comes to, pulls his arms free and clutches the board's edges. In the next, closer shot, he's unconscious again, wakes up and repeats the grabbing-the-board motion. (00:07:30)
Answer: There wasn't one singular city used. It was shot in and around various locations in California, plus on a lot of sets and backlots. Ex. Some scenes were shot in LA, some in Pasadena, some in Santa Barbara, etc.
