Character mistake: When Jeff is on the phone to Washington, thanking them for arranging the Sentinel to pick up Thunderbird 4 and take it to New York, the man in Washington replies, "That's the least we can do, Mr Tracy." So at least one person knows that Jeff is involved with International Rescue. This betrays the secrecy of the organisation that Jeff himself demands.

Character mistake: Inside the Mobile Radio Control Police Van, there is an "Ultra Short Wave" device that is labelled "Ulta Short Wave."

Revealing mistake: When the Firefly is knocked back by an explosion after putting out the oil well fire, we see a view of Virgil in the cockpit. You can tell this shot is reversed as there is a back-to-front IR logo.

Continuity mistake: When Scott is at his control panel at the crash site in New York City, the top of the microphone keeps disappearing and reappearing between shots.

Continuity mistake: At the beginning of his journey on the Sentinel to the rescue site, Gordon has the beginnings of a beard on his face, but in subsequent shots he is completely clean-shaven.

Revealing mistake: In the scene when we the Fulmer Finance building starting to collapse, a wire can be seen attached to the top of the building.

Continuity mistake: In its first appearance when it attacks Thunderbird 2, the Sentinel is rapidly moving at all times. But when we see the missiles being prepared for fire, the craft is holding still. You can tell this as the clouds in the sky are not moving.

Other mistake: One of the puppets used in this episode is called "Joe" and is a TV Cameraman. As this particular puppet falls into the cavern, the back of his head can be seen to fall off.

Continuity mistake: When we see Virgil in his sick bed after crash landing on Tracy Island, he has a bandage around his head and there is a blood stain on the bandage above his left eye. But when Gordon comes into his room to watch the moving of the Empire State Building on television, the blood stain has disappeared.
Visible crew/equipment: After Central Control announces the countdown for Red Arrow project is now operating, when it cuts to the tower the order is given to "begin photographic alert procedure," and in the next shot a human finger (with a long polished fingernail) is visible behind the seated male marionette, as he leans forward. Continuity mistake: When The Hood drives his van off the broken bridge whilst escaping the police, watch carefully; the van bounces off the rocks on the other side and starts to fall roof-first into the river below. However, when it cuts to the van falling into the water it falls in upright. (00:42:00)
Continuity mistake: Before Jeff pushes the button on his desk to activate Operation Cover-Up, you can see there is a dark blue line around the edge of his sleeve, but in a close-up of him pushing the button, the line has disappeared. It reappears in the very next shot.
Revealing mistake: In the shot that shows Jeff Tracy reading a newspaper, the date shown on the paper is 1964, however, the year the series is supposed to take place is in the mid to late 21st century.
Character mistake: Tim Casey is a Colonel, but Scott refers to him as a General in one scene.
Continuity mistake: When the hood parks his car to watch the first Red Arrow crash, there is a white and pink car parked beside him. When he leaves, the white car is still there, but has changed parking lanes and the pink car is replaced with a red car.
Continuity mistake: In one zoomed out shot of Virgil standing beside the tower with the Thunderizer moving closer towards him, he is no longer wearing his raincoat.
Continuity mistake: After Lady Penelope rescues Brains from the psychiatrist's office, when she holds out her communications compact to message Scott it's in her left hand, but in the closeup it's in her right hand and the color of her sleeve also changes. (00:45:50)
Continuity mistake: During the rescue operation, Thunderbird 2's wreckage grabber changes from being semi-circular with flat edges as TB2 flies over the water to being angular with teeth as it is being dropped down to the rocket.
Character mistake: When Lady Penelope visits the psychiatrist in his office, she addresses him as "Mr. Korda" instead of "Doctor".
