Audio problem: As Kirk low crawls behind some boulder and grass, his communicator flips open and closed. If it was opened, the sound of the channel opening should have been heard, but wasn't. (00:36:35)
Audio problem: When Uhura apologizes to Spock for interrupting him playing his instrument, her word is cut off mid speak between cuts. She says "I'm sorr-." But it's close enough it easily got past the editor or the editor cut her word off too soon not realizing. (00:09:59)
The Return of the Archons - S1-E22
Audio problem: In the transporter room, as Kirk is talking to Sulu, the door opens, McCoy comes in, and the door shuts. Trouble is that the characteristic sound of the door is missing.
Who Mourns for Adonais? - S2-E2
Audio problem: At the temple, as Kirk goes to call the ship, he flips the communicator to open it, as he usually does. However, he has to flip it twice to get it to open, but the sound of the channel opening is heard from the second he starts.
Audio problem: About a minute or two into the episode, Captain Kirk says "That's incredible" but his mouth doesn't match the words.
Audio problem: As Commodore Stocker freezes up on the bridge, before the cut to sick bay, the bridge is shaken by a Romulan blast, but the sound is absent.
The Ultimate Computer - S2-E24
Audio problem: After trying to change course, Sulu calls Kirk. Kirk says "Kirk here", and then you hear the click of the comm button being hit.
The Enterprise Incident - S3-E2
Audio problem: During the scene where Kirk is talking in the briefing room, the director was obviously not pleased with how James Doohan said his lines. Due to budget constraints, instead of re shooting the scene, they simply replaced Mr Scotts dialogue with another one. You can see it since at one point his lips are completely out of sync, even on the remastered edition. (00:06:40)
And the Children Shall Lead - S3-E4
Audio problem: When Spock asks Kirk, "Why are they bothering Starfleet?", there is a loud buzzing noise twice as they are focused on Kirk's face.
Audio problem: The bartender comes over and brings whiskey and five glasses, but we hear six glasses hit the table.
Audio problem: Spock uses the intercom to call Mr. Scott who is in engineering. As the shot cuts to a side view of Scotty standing at the controls, he answers "Scott here" but his mouth or lips never move. (00:44:24)
Audio problem: When Chekov goes berserk on the bridge, he emits a scream and shoves Spock. When Spock holds Chekov's face, the closeup on Chekov shows a scared look, and the original scream is played again, but Checkov's mouth doesn't match.
Audio problem: Just as Garth brings Spock back and releases Kirk from the cell, "inviting" them to dinner, there's an odd buzzing/rattling noise in the audio that shouldn't be there. (00:11:30)
Audio problem: When Kirk demands to know who the Troglytes are, his voice is dubbed over: his lips don't move at all. (00:04:05)
Audio problem: In Zarabeth's cave, when McCoy says, "You've been dishonest with me, Spock," his lips are completely out of sync with his words. (00:42:00)
Answer: After causing Norman to overload, all of the other androids shut down. The same could be said for the androids on the Enterprise.
