Continuity mistake: When Chris and Bianca go to the manor to get Chris to go to the past, which would end up him appearing in "oh my goddess"; he's only wearing a black shirt and pants. When he arrives to the past in "oh my goddess" he's wearing a jacket, red shirt, and sunglasses. He wasn't wearing that in Chris-Crossed. And he didn't have time to change to that outfit.
Witch Wars - S6-E21
Visible crew/equipment: While the demons are watching Witch Wars at the bar, when the show's host, Corr, is holding an athame he tells viewers the winner will be awarded a serious power boost, then just as Corr says, "Since the shapeshifter..," in the shot of the demon bar patrons, we see the T-mark (it is not part of the Witch Wars show) taped to the floor under the table. That T-mark is visible again in at least three following shots. (00:23:35)

Visible crew/equipment: After The Tribunal reverse the decision about Darryl he reappears at his desk and he hugs Sheila, then as the camera pans around to reveal The Cleaner standing in the doorway, we can see the white tape mark on the floor behind his feet. (00:39:00)

Continuity mistake: When Leo and the sisters are in the alley with darklighters, Phoebe falls from the roof and her high-heeled boots become flat shoes. (00:14:40)
Continuity mistake: When Chris tells Phoebe that searching for Jinny at the dig site may be a trap Chris is wearing a zipper hoodie, but in the next overhead shot just as Chris sees the hole containing bones Chris has on what looks like a leather jacket, and then it's back to the zip hoodie. This is odd, because it's not until later after Leo heals Chris, that we even see Chris in the leather jacket. (00:03:10)
Continuity mistake: When Phoebe is told that Jason asked not to be disturbed she decides to go in anyway, and right before she enters Jason's office the lace-up on the back of Phoebe's top is very loosely tied so we see a lot of skin, but when she's inside Jason's office the lace-up is tied quite tightly so only a bit of skin is seen. Continuity mistake: When Paige and Phoebe throw the potion at Darryl to borrow his soul, Darryl's physical body lies face-up with nothing on him, but in the closeup the necklace with Darryl's badge is lying neatly atop his chest. (00:36:15)

Revealing mistake: When the sisters summon the Trok demon Phoebe is wearing a bikini top, and as the demon blasts them all backwards into the wall, Phoebe is wearing something around her waist for the stunt. (00:07:20)
Continuity mistake: When the sisters first see the lady in the lake, the demon off to the side throws a knife into her back, but the knife hits completely straight, as it would if the demon had been directly behind her.
The Courtship of Wyatt's Father - S6-E16
Revealing mistake: Just after Piper and Leo are taken away, Paige orbs down to the ground and runs up to Pheobe, places her hand on her, and shows a blood-covered hand showing that Pheobe's been injured. But before it's shown, while Paige is running up to Pheobe, you can see Paige deliberately keeping her hand closed and facing away from the camera, because it's already there when she orbs down.
Continuity mistake: When the main Calloway throws the clear bottle with the green potion at one of the enemies, the shot cuts and it's now become a white bottle.
The Legend of Sleepy Halliwell - S6-E14
Continuity mistake: When Phoebe comes out of her vision quest the third time, the shaman holds out the potion cup by the handle, but in the next shot, she's holding it by the bowl. The cup also appears to jump from her right hand to her left. (00:30:35)
It's A Bad, Bad, Bad, Bad World (1) - S6-E22
Other mistake: Phoebe says there's nowhere to park except blocking the neighbor's driveway. We can see in the opening shot that there is no-one parked in front of the manor, and we see spots across the street. There is also enough room to park where she is without blocking the driveway. At the end of the episode, despite not moving it and also saying that she hasn't, it's not blocking the driveway any more. The patrol guy still shoots her. It's not illegal to park there unless it's blocking the driveway.
The Courtship of Wyatt's Father - S6-E16
Other mistake: When the Darklighter shoots Leo in the attic on the Ghost Plane, the arrow goes all the way through him. He wasn't shown to be using anything other than a normal Darklighter Crossbow, and there wasn't any evidence that, since Leo and Piper were still alive, that the physics of the Ghost Plane are any different from the Normal Plane. Yet, in every other instance of someone getting shot with an arrow from a crossbow, it never, ever penetrates the person's body, and always get stuck.
Used Karma - S6-E13
Continuity mistake: In the scene when Paige and Piper are doing first aid on each other, Piper makes quite a production out of the business of peeling the backs off band aids to apply to Paige's arm - but when she slaps it on Paige's arm, it's a non-adhesive white 4 x 4, which Paige has to hold on her arm with her other hand.

Continuity mistake: At the beginning, Piper is wounded on her forehead by the demons. When they orb out and orb into the kitchen Piper doesn't have a wound on her head anymore. (00:03:25 - 00:05:40)
Valhalley of the Dolls (2) - S6-E2
Continuity mistake: When Jason is escorting Phoebe into his office her left arm is down by her side in the rear view. When the shot changes to the front view, her left arm is across her stomach.
Chosen answer: Her agent/manager negotiated a special billing as part of her contract. It distinguishes her from the rest of the cast, probably when she became a producer on the show.
