Continuity mistake: When the gang are at Champions on Ice, Karen gets out a bottle of champagne and a glass a starts pouring herself a drink. As the shots change, the bottle and glass keep swapping hands, even as she is still pouring. (00:13:45)
Plot hole: Will and Jack are out on the patio talking about the first time they met at a party (implying in college) and how Jack convinced Will to come out, but in S1E6 Jack talks about Will's favorite book in elementary school. Suggested correction: I just re-watched the scene you referenced from Season 1. Jack is talking to Grace and is throwing out random Will trivia. He never said he knew Will in elementary school, just that he knows what Will's favorite book was when he was in elementary school. There is no plot hole with this statement.
Factual error: When Jack is speaking to his mom on a mobile phone outside of his apartment door, he tells her that he will take her to Souplantation after she comes over that afternoon for a blowout. Jack's apartment is in Manhattan. Souplantation is a restaurant chain that only exists in California. (00:19:35)
Other mistake: During the court hearing the cop Vince mentions looking in the trunk of Karen's car. Nowhere in the scene prior when he stopped them did he look in the trunk.

Continuity mistake: When Grace meets Leo in the coffee shop and is going to tell him that she's pregnant, during the whole conversation between them the amount of hair hanging over Grace's left shoulder keeps changing.

Continuity mistake: When Grace and Sam are talking in the attic and Sam is saying that he is terrified of being single, etc., his arms keep alternating from crossed in frontal shots to uncrossed in shots from over his shoulder. (00:19:05)
