Continuity mistake: A taxi is pursuing a Mercedes sedan. At one point they collide - taxi left side and Mercedes right side. Later in the chase, the scrapes and tire marks on the Mercedes are on its left side instead of its right. (00:08:37 - 00:09:39)
Continuity mistake: Jason and the team track the bomber to his mansion and isolate him in his hiding spot. Jason opens the door outward (toward him). In the next scene when the team streams into the room the door is open inward.
The Strength of the Wolf - S3-E4
Audio problem: One of the team members shoots a sentry on board a ship off a ladder attached to the deck hanging off the side of the ships hull. He shoots an unsurpassed Glock and the shot sounds suppressed. A muzzle flash can be seen too, another indicator it's unsuppressed. Their element of surprise would have been totally eliminated had this really happened. They've used suppressors before on their sidearms - doesn't make sense why they wouldn't now. (00:31:00)

Factual error: The Seals use Surefire suppressors which reduce flash by 99%. As the suppressors get warmed up the flash slightly increases. In the show the muzzle flash is that of unsuppressed weapons. (00:32:20 - 00:33:00)

Continuity mistake: The team and the hostages escape from the compound in an SUV and a school bus with the SUV in the lead. They are being pursued by a small truck with a mounted machine gun which starts firing on the school bus. They switch positions, moving the SUV behind the bus so that they can protect the hostages and return fire. The next shot in the command post via a drone shows the original lineup, with the school bus behind the SUV.
All Along the Watchtower: Part 2 - S3-E6
Visible crew/equipment: When they walk through the hallway towards the second building, a crew member can be briefly seen.
The Ones You Can't See - S3-E7
Other mistake: Before the Seals went on their counter-sniper operation they were briefed that the enemy snipers were going to be using SVD-63 Dragunov's which have a range of 800m. The two enemy snipers who were killed never had Dragunov's, they had standard bolt action type rifles without suppressors, hard to identify a specific brand. They engaged the Seals from a distance with unsuppressed weapons over a crowd of people and the crowd didn't hear a thing. This is a falsity, a 7.62mm shot can be heard for 3km. (00:37:26 - 00:38:00)

Deliberate mistake: In a previous episode the Seals conducted a vehicle interdiction snatch and grab because the HVT they were pursuing was said to have lived in an apartment building with a lot of security, cameras, doorman, etc. Part of the team in Mexico now goes to the HVT's two story house to find her dead. What happened to the building she was residing in? (00:11:00 - 00:12:00)
Unbecoming an Officer - S3-E10
Continuity mistake: Jason is in bed with his girlfriend lying on opposite ends of the bed. The girlfriend's feet are exposed. When the camera pans up to her face and back down, she only has one foot exposed. (00:02:00)

Visible crew/equipment: When Trent is approaching the door you can see a GoPro attached to the barrel of his rifle, on the left side of the weapon. (00:08:13)
Character mistake: Sonny and a Turkmen soldier are having a competition shootout, and they're both using CZ75 9mm pistols. These firearms are very high end and are made in Czechoslovakia. Sonny says "I have this little Russian pea-shooter." The place of manufacture is stamped on the frame and its Czechoslovakia. (00:20:00 - 00:20:30)
Other mistake: Ray and Clay are utilizing sniper rifles to combat the hostile enemy. Their shots are both shown through a thermal optic view, however both of their rifles only have standard day optics. For a thermal view to be operational a separate thermal optic needs to be clipped on to the front of the day scope/optic to provide a thermal view. A day scope/optic doesn't magically become thermal. They mount onto the top rail of the Picatinny Rail. (00:29:00 - 00:29:40)
Character mistake: Lindell is referred to as Captain Lindell; however, on his office wall, he has a Meritorious Service Medal Award that references Captain Greyson Lindell, USMC. The fact Lindell is a Marine, he should be recognized as a Colonel and be wearing a USMC uniform, not a U.S. Navy uniform. (00:26:43)
Factual error: A US Navy Lieutenant Commander (Chaplain) is shown wearing a metal chaplain badge on his working uniform. This is not authorized. Working uniforms require cloth badges. (00:09:40)
God of War - S4-E1
Other mistake: The team is loading their P-Mag type magazines with 5.56 bullets, however the bullets are "glued" into the couch facing down facing down, however they do not have a tip. Ammo is loaded from boxes, or stripper clips. I've never seen ammo inserted into a couch to be placed in magazines. (00:08:17)
Plot hole: In episode 3 Ray and a CIA agent meet a US Ambassador to Tunisia to discuss placing a Seal team in Tunisia to combat the recent wave of terrorist bombings. The Ambassador was against boots on the ground. In episode 4 when Bravo team arrives to find captured Ray, they land at an airbase in Tunisia and enter a portable type building, on the wall of said building are US Military patches such as 82nd Airborne. Did the ambassador forget America has portables there for their troops?
Other mistake: Jason Hayes is arrested by a NIS (Naval Investigative Service) agent and placed in handcuffs. Still in handcuffs, Jason is walked into the United States Naval Consolidated Brig by Shore Patrol. The handcuffs are removed and Jason empties his pockets. Jason places a knife in a bowl. There's no way a prisoner would be arrested or allowed into a jail or brig without being searched, and there is no reaction to the fact there is a weapon and a prisoner had access to it. (00:01:01)

Factual error: The grave headstone for Benjamin Bellerow states Master SGT US Navy. There are no Sergeants in the Navy, only Chiefs. If the rank was correct it should have read US Marine Corps, US Army or US Air Force. (00:19:08)
Character mistake: During the CO's meeting, the Captain made a mistake by addressing Commander Blackburn as "Lieutenant Blackburn." Even if he was demoted to Lieutenant Cmdr, you still suppose to address that rank as Commander or Lieutenant Commander. (00:04:44)
