Continuity mistake: When Fry first meets Leela, there's a panel on the wall beside him. It has one red button at the bottom. In subsequent shots it has two buttons. (00:03:50)

Continuity mistake: The windows at the beginning of the episode are inconsistent. Sometimes they're transparent, sometimes opaque. As in the taxi, the newspaper stands and the shop windows. (00:00:30)

Genesis of the Daleks - S12-E4
Revealing mistake: Whenever there's a close-up of Davros, it's very easy to see that there's a gap around his mouth where the mask should blend in with the actor's face, but doesn't. This mistake was rectified, incidentally, in Davros' next appearance in "Destiny of the Daleks."

Trivia: The actor who played Davros, Michael Wisher, always wore a kilt to the studio, as he preferred it to trousers. It is never seen, of course, as his lower body is always hidden inside his Dalek chair. (Source: DVD commentary).

Deliberate mistake: Chief of Security Nyder wears an iron cross on his uniform which mysteriously disappears in episode 3 - apparently the producers felt it was too close a reference to Nazism so it was removed from the show. (Source: DVD commentary).

Doctor Who (1963)
Revealing mistake: In episode 6, when the Brigade Leader shoots the Primord Stahlmann, he falls down but no bullet holes are made in his costume. Thanks to Producer Barry Letts for spotting this one on the DVD commentary. (00:17:00)

Other mistake: A few times during the whole show there are certain shots where the corners of the screen are blacked out, as if a circular lens cap has been fitted to the camera. There seems to be no logical reason for this, therefore it has to be a mistake. One example is when Jonas is leaving his wife for the last time and he says 'there's a big battle brewing'. (00:07:25)

Doctor Who (1963)
Continuity mistake: In episode 4, the Doctor's bow shrinks from when he talks to Liz Shaw while in prison, to when he stows aboard a jeep later. In the DVD commentary, producer Barry Letts blames himself for this mistake. (00:13:00 - 00:19:50)
