Genesis of the Daleks - S12-E4
Revealing mistake: During the Doctor's early conversation with the Time Lord, there is fog swirling around them. When the Doctor agrees to take on the mission, it suddenly appears very concentrated around the tip of the rocks behind them, as if generated from a source behind the rocks. Normal fog would not do this, and it's clear that dry ice was used as the fog effect (DVD commentary confirms this). (00:02:45)

Continuity mistake: When Sarah falls from the scaffolding, a plank of wood saves her from death. Trouble is, she fell a foot from the outside of the scaffolding and most of the plank was inside the scaffolding, therefore her feet would have hit it and she could only have landed with her head pointing inwards. But when we see her, her head is pointing outwards. (00:01:25)

Continuity mistake: After Zapp Brannigan shows off his 'big book of war', he returns it to a pouch at the side of his chair. Two shots later there is no sign of the pouch or the book. Also, when he gets out of the chair, there are no buttons on the armrests, but when he returns to it later, there are two on each side. (00:07:20)

Continuity mistake: When the team are gathered round the table at the beginning of the episode, note their positions when the video screen is turned on. When they see Fry drying his hair, Leela and Bender have each moved up a chair, and Zoidberg has also moved, and seems to have taken half of his previous chair with him. (00:01:20)

Continuity mistake: Zapp Brannigan initially has about a dozen pink pillows on his bed, but when Leela pulls the plug on him, there are much less. Then in later shots there are more again. (00:10:55)

Continuity mistake: When Bender sits beside Fry on the couch, Bender's sitting at the end with his arm on the rest. When Fry says 'Leela, we're trying to watch TV' Bender's moved to the middle of the couch. The garbage next to Fry and the contents on the table also change in this scene. (00:03:35)

Continuity mistake: When the spaceman is delivering the 'Planet Express' package, the colour of the rocket on the box changes from black to green. The same thing happens to the spaceman when he's picked up by the large bird - the logo on his backpack changes colour. (00:00:30)
