Revealing mistake: Just before the Axeman throws his axe at Claire and Alice, we get of shot of Claire turning around. But pay close attention to the wall behind her. You can already see the crack which is going to hold up his axe before he throws it. And it isn't an old crack, because when Alice is about to take a shower, all the walls are perfectly fine. Continuity mistake: During the fight scene with the Axeman, Claire slides between his legs while gunning him down with Alice's shotgun. But watch his axe. In the far shot he is holding it up, like he is ready to strike, but in the shot where he is falling, the axe is leaning on the ground all of a sudden. (01:04:15)
Continuity mistake: After the Tower Swing, Alice lands where she started out: in front of the garage with the UPV. Note the sign with the gun on it. But the door right next to it is closed. Behind that door is the UPV. Since everybody was in somewhat of a rush, it doesn't make sense that anyone should have closed it.
Revealing mistake: When the Umbrella guards are lining up in front of the elevator, we get a few close-up shot of the level counter. During those close-ups, you can see the ribbed rear wall of the elevator when the dial is still only at level 3. Compare the wall to when the doors open, it is exactly the same. This means that the elevator isn't moving at all, and that it is just the dial. (00:07:05)
Continuity mistake: Alice is in the shower and draws her gun from the holster on the pole. She takes a couple of steps forwards, camera cuts, and she's suddenly much further forwards.
Continuity mistake: Kim Yong is killed in the shower block and his corpse falls to the floor but is never seen again. (01:02:30)
Character mistake: Chris explains about the APV being behind the door and specifically states it has 16 wheels. When we see the APV it has 4 wheels on each side. Meaning 8 in total. For it to have 16 wheels as stated, all the wheels will have to be doubled up. However its clear that at least 2 separate wheels are single. Since Chris is right with everything else he has said in regards to the prison, there's no reason he would be wrong about this one thing. (00:56:00)
Continuity mistake: When Crystal is emerging from the water, she is almost out of the water completely in the long shot. As the camera changes to a close up, she's much further back into the water. (00:54:40)
Continuity mistake: Alice does her pass of the prison and Kim-Yong can be seen hitting the deck away from the white lines of the help us sign. However in the wide shot he is on a white line. (00:30:50)
Continuity mistake: Wesker shoots the supervisor in the head for questioning an order, The guard falls to floor and his position changes several times depending on the shot. Note his head and pool of blood in relation to the lines on the floor. (00:10:30)
Continuity mistake: Wesker arrives at the command post to get a report. Note where he is stood however as his position in relation to the black lines on the floor is constantly changing. (00:06:20)
Revealing mistake: When ripping the cables up on the roof to make an improvised brake for the plane, he tugs the cables so hard they actually hit the camera resulting in it wobbling before cutting away. (00:31:00)
Continuity mistake: When inspecting the plane for damage, the propeller changes position without being touched. The side by Alice goes from the 8 o'clock position to the 10 o'clock position. (00:34:30)
Continuity mistake: In Extinction, we see Kmart be handed the Alaska book and say "You're not coming?" to which Alice replies "Take care of the others." In the flashback seen here however, Alice replies "Take care of the others, Kmart".
Continuity mistake: The bullet holes in the doors the two Alice clones dive through near the start are constantly changing place. (00:11:05)
Revealing mistake: When Wesker flips Claire and Chris during their fight, in the brief sequence from the side when she lands on her back, a TCR timecode is visible in the lower-left of the screen for a single frame before it cuts to the sequence with Alice cornered by dogs. TCR timecodes are meant to be removed when a film is finished. (01:18:40)
Continuity mistake: In the after titles sequence, when Jill is addressing the group in the plane, she is walking with her hands on her back. When she turns around, heading towards the tail, in the long shot her hands are on her hips.
Continuity mistake: When the elevator hits the water, it starts to tilt over to one side. This contradicts the shot which shows it falling, where it fits very snugly in its shaft. (00:59:30)
Other mistake: When we see a shot of the plane flying away from under the camera through the mountains, the 2:40 crop (letterbox) is drastically warped in the upper right half of the image. Spotted on the Australian Blu-Ray release. The DVD copy does not have this error. (00:19:20)
Continuity mistake: When Alice is shining her flashlight into the tunnel, note that the shower column is on the right side of the tunnel entrance. In the shots on the outside of the tunnel, that same column is on the left side of the entrance. (01:04:30)
