Visible crew/equipment: When Jamie Foxx enters the building where the girl works, he finds the dead guard - right after it he runs alongside a marble wall where you can see the cameraman running beside Foxx. (01:37:15)
Continuity mistake: When we first see Vincent in Annie's building, he is in an office to the left side of the building. You can tell this because there were offices down and to the right. However, when Max is telling Annie "now" there is an office below her on the right side of the building. (01:39:00)
Revealing mistake: In the train scene if you look closely at Vincent's ear as he steps off the train ready to shoot Max and Annie, it looks like he has an earpiece or some kind of small blood pack in his ear. (01:44:05)
Other mistake: In the scene where Max and the lawyer board the train, they go down the stairs for their train. Vincent is a level above and gets on the same train somehow. (01:46:00)
Continuity mistake: When Max and Annie are walking away from the train, in a shot from behind Max has his right hand on Annie's back, but in the following shot from the front Max's right arm is all the way around Annie fixing her jacket. (01:47:35)
Continuity mistake: When Vincent is shot in the face, the bullet left a long gash below his cheek across his jaw line. A couple of scenes later, when he's in the train the gash is gone. All we see is make up which is supposed to be blood.
Continuity mistake: When Vincent faces Annie for the first time in her office, her face and forehead are all sweaty and her hair is a mess and all over her face. In the following scene where Max and Annie are in the elevator, Annie's hair is back to normal and is very well combed. Also, her face is not sweaty anymore.
Continuity mistake: In the scene when Max and Annie get off the train after Vincent dies, it is pitch black. After the train moves away, and Max and Annie walk down the street, dawn has broken.
Continuity mistake: When Foxx and Cruise are in the lift with the cop in the hospital, the door to the lift closes all the way to the right. When they get out, the door closes in such a way that two doors meet in the middle.
Factual error: Toward the end, when Vincent et al board the train, they're on the southbound Metro blue line out of 7th Street station. The next stop should be the Pico station, next to Staples Center. But we see them get off an eastbound train along the green line, half an hour's ride and a transfer away.
Continuity mistake: In the club Fever, one of the Korean bodyguards gets up off the ground and runs to restrain Vincent. As he gets up, the people around him are dancing when they're supposed to be running around panicking. When Vincent shoots the bodyguard holding the MAC-10, another bodyguard in a white suit is visible in the background already trying to pull Peter Lim out of the booth. The bodyguard does not run to Peter Lim until a few seconds later in the scene.
Continuity mistake: When Vincent is talking to Daniel (the trumpet player), Daniel's upper lip changes from sweaty to dry at least 3 times.
Character mistake: In the scene where Detective Fanning and the other cops are investigating the first murder, one cop tosses a shell casing in a baggie out the window to Fanning. The cop then remarks, "You can still smell the cordite." This is incorrect because cordite has not been used as cartridge propellant since approximately the end of World War II.
Continuity mistake: After running out of the hospital, Vincent and Max face off on the foot-bridge over the freeway. As Vincent moves toward Max, in the long shot you see him move the gun from his right hand to his left. In the close-up, it stays in his right hand.
Factual error: The Blue Line train actually makes many stops with stops occurring approximately every 2 min. (with or without passengers) The scene at the end in the film lasts many minutes without a single stop.
Continuity mistake: After Max and Vincent leave the first hit, while they're talking about why Max cares about the death of "some fat Angeleno," their cab is being followed in rapidly successive shots by a silver car, then a black SUV, then the silver car again, then the police cruiser.
Continuity mistake: When leaving the club Fever (I think) the taxi driver is so shaken he drives off and hits two cars parked to the left, leaving them badly damaged. Later, just before he flips the taxi, you can see that both sides are unmarked, which seems unlikely.
Continuity mistake: After they leave El Rodeo there is noticeably more blood on the roof of the cab and in the cracked glass of the windshield, so much so that it is running down toward the hood.
Plot hole: Near the end Max calls Annie, using the number on the card she gave him, to warning her about Vincent's arrival. But why is there on the card the number of the law library on the 16th floor and not Annie's office number or her cell phone number?
Suggested correction: The number on her business card is her office line. A lot of office phones are set up so they ring in multiple places throughout the office. A light will blink on the phone letting you know which line is ringing. The line is connected to her office phone, which we see Vincent looking at in her office when she was on the phone with Max, which then gives away her location. This is a very popular way of having phones set up in offices, as it eases communication between co-workers. If one of her co-workers wants to talk to her while she's in the library, they can simply dial her line instead of going up two floors. Furthermore, it is also possible that her cell phone number could be on the card. But Max tried the office number first. He would likely have tried the cell number next, had she not answered.
Continuity mistake: When Vincent shoots the two guys in the alley who stole his briefcase, there is no damage on their bodies whatsoever from the gunshots. Despite shooting both numerous times you can see there are no holes or blood on either of them.
Suggested correction: He could, in fact have an earpiece/hearing aid in place, and what with the wild night he's been having, the accident and all, a blood pack is not out of place either.

It's visible that just a little amount of blood dried up in the ear. Not any earpiece or blood pack.
