Continuity mistake: When Kyle is running after Andy against the factory, we see Andy approaching a huge truck. However Andy passes the truck several times between the shots. (01:02:20)
Continuity mistake: After Kyle takes the sock out of Andy's mouth, she places it on the bed. In the next shot it has gone. (00:29:40)
Continuity mistake: Look at Andy's feet when he was freed when Kyle comes in through the window, he has no shoes on, but when he chases after Mr. Phil Simpson he gains a pair of shoes. (00:30:00)
Continuity mistake: When Chucky pretends to be Tommy, a small drawn on mole, under the left side of the dolls' mouth is alternately there / missing. The position of the dolls' hair keeps altering as well. (00:25:00)
Continuity mistake: When Miss Kettlewell is reprimanding Andy in class for disturbing her lesson, the shot alternates from behind her to in front. During each shot change, the position of the book she clutches in her arms keeps changing. (00:33:10)
Continuity mistake: When Kyle climbs into Andy's room, she unties his hand from the skipping rope, and Andy punches Chucky, who falls down onto the floor by the side of the bed. Yet when Phil comes in, he picks up Chucky from the floor by the end of the bed. (00:29:30)
Continuity mistake: When Joanne, Phil and Andy are in the car after they have fostered him, the shots alternate from the backseat to the front, during the scene. During the shot changes Joanne's purple jumper sleeve keeps differing in how far it is rolled up. (00:09:15)
Continuity mistake: The toys in the back seat of Mattson's car change from when Chucky is tossed in, to when Chucky comes alive. (00:14:45 - 00:16:05)
Continuity mistake: When Miss Kettlewell is about to check the children's homework sheets, she puts her glasses on. Then when she discovers the profanity written on Andy's work, you can see her glasses hanging down by her chest. They're back on in the next shot. (00:33:35)
Continuity mistake: When Andy approaches Chucky posing as Tommy, near the foot of the stairs, the doll's freckles are faded in some shots, and more prominent in others. (00:25:00)
Continuity mistake: When Joanne is singing to Andy during the thunderstorm it pans to the banister to show Chucky standing at the foot of the stairs. In this shot Chucky tilts his head to his left. In the immediate close-up his head is tilted to his right. (00:21:15)
Continuity mistake: When Chucky walks out of the closet saying "You've been very naughty..." there is no gap between the clothes hanging on the rack behind him, where they were pulled apart previously by Miss Kettlewell. In the next shot, there is again. (00:37:00)
Continuity mistake: Throughout the classroom scene, there is a row of desks which are right by the wall, which keep either disappearing or moving. (00:34:40)
Continuity mistake: When Kyle and Andy are in the garden, we see Chucky sitting on a flower cart. In a wide shot he sits in the middle of the cart, in a closeup he's on the left. (00:26:40)
Continuity mistake: In the school cloakroom above the coats, is a shelf of dodgeballs, footballs and basketballs. Look at the arrangement of the balls when Miss Kettlewell enters the cloakroom. When Miss Kettlewell is stabbed, and crashes into the desks, you can see in the cloakroom that the arrangement has totally changed and additional items have appeared. (00:36:25 - 00:37:05)
Continuity mistake: When Grace falls to the floor dead, photocopies fall over her body and face. But when Kyle opens the door and sees her, there are no sheets of paper on her face anymore, just two sheets on her body. (00:59:00 - 00:59:55)
Continuity mistake: When Ms. Simpson and Andy enter Kyle's room, Kyle's lighter is lying on the table beside the cloth. When they leave, the light has moved onto the cloth. (00:12:30 - 00:13:00)
Continuity mistake: Near the beginning, the Chucky doll causes a worker to be electrocuted and fly backwards. As he somersaults through the air, and through the window, it is clear that in this angle he will land on his front. But in the next shot we get a close-up of him lying on his back. (00:05:55)
Continuity mistake: When Kyle has dropped the knife to the floor she is standing by a mirror looking down to her right for the knife before reaching for it. It cuts to a closeup of the knife and Chucky grabs it before her. It cuts back to Kyle who gasps in fear, but now she is looking more to her left like everything is happening on that side. (00:52:50)
Continuity mistake: Miss Kettlewell gets stabbed, and goes flying out of her cupboard. As she flies into the desks and falls she is wearing tights, but in the previous shots she wasn't. (00:36:55)
