Visible crew/equipment: At the beginning when the robot is lifting fingerprints from the suitcase, you can see a person walking around in the background-something not likely when there is a bomb threat.
Visible crew/equipment: When Ned walks upstairs to check on the lady before the bad guy gets killed, behind him is a circular window. A reflecting light can be seen in the window along with what looks to be a boom microphone.
Visible crew/equipment: Toward the end of the episode you can see a guy with a camera reflected in the side of a silver car.
Rumpole and the Fascist Beast - S2-E4
Visible crew/equipment: While Rumpole's wife prepares the canned soup in the kitchen, several equipment shadows are moving on the wall at the top of the screen. (00:06:40)
Visible crew/equipment: In the flashback of the murderer slamming the car door on the victim's neck and breaking it, we get a clear shot of the cameraman and his camera in the car door (CSI's favorite blooper).
Visible crew/equipment: The Great Santini makes a vase of flowers appear on a table. Before he holds up a cloth to conceal the vase as it appears, the tips of the flowers can be seen beyond the far edge of the table. As he pulls the cloth away, there is a quick glimpse of the hand of the crew member who put the flowers on the table. (00:11:00)

The Beautiful Day in the Neighborhood - S5-E4

Visible crew/equipment: In the rec hall, when Kensi feigns sickness while posing as a cadet the gunman jumps off the stage, and as Kensi bends over the small rectangular bulge of her mic pack can be seen at her back. (00:30:25)
Chapter Five: The Flayed - S3-E5
Visible crew/equipment: While Bruce chases after Nancy and Jonathan they hide in the OR, then Nancy picks up the red phone and shouts, and in the following closeup of the ceiling mounted domed light, the upside-down reflection of a crew member wearing shorts and sneakers can be seen quickly. A few minutes later, after Jonathan jabs scissors into Tom's throat, when Jonathan leans back out-of-breath there's another closeup of the surgical light, and we can see the upside-down reflection of the cameraman's arm and part of the camera as it moves closer, then it's visible again a few shots later. Visible crew/equipment: In the scene where Jarod is being thrown into the swimming pool by Zed, the gang leader, as a sort of initiation, there is a shot where Jarod is tied up in chains sitting in a chair at the bottom of the pool. Another character is set to throw in the key just to the next of Jarod and he is supposed to fall over and get it. In the shot just before he falls over, you can see a crewmen's arm holding a chain attached to the chair in order to help Jarod fall over.
Rendezvous - S2-E15
Visible crew/equipment: In the scene where Bud is shredding the satelite photograph, when he turns around to talk to Harriet, a microphone dips into the scene for a quick second.
Visible crew/equipment: As the energy being moves down the city sidewalk, we see its shadow on the concrete. Right beside that, however, we can also see the distinctly-shaped shadow of the camera. (00:35:00)
Visible crew/equipment: When the parish priest is preparing for the Easter Pageant and gathers up the bundle of sticks, the microphone dips into the shot at the top of the screen. (Aired version only. Mistake was digitally removed on the dvd.) (00:16:05)
