Revealing mistake: Paul Devildis calls the bank to discuss his restructuring plan. During the shots of the two bankers, on the desk is a notebook-sized, plain brown box. It is very distinctive as it is blank except for a single square white label on the front. Later in the episode Paul delivers his restructuring plan with the bomb inside to the bank. It is in this same box.
Water's Edge - S3-E3
Revealing mistake: The murder victim's skeleton, which Rusty finds in the attic, has a clear cut all the way around its skull. It also has metal pins holding together what turns out to be rubber bones: one of the ribs wobbles up and down when Rusty brushes against it with the cash box. (00:42:00)
Revealing mistake: It's a valiant effort, but the scenes of Sherlock & Irene in a London street are shot in New York. Not only is the architecture wrong but you can see a U.S. pedestrian crossing signal over Irene's shoulder. (00:16:15)

Revealing mistake: When Kieran looks at his phone in the car, the image of Emma calling him is upside down. The image was added after the filming was done.
Revealing mistake: Near the end of the episode, when Dr. Ogden and Murdoch look closely at Lukas' dead body (right before the candle scene), the "corpse" takes a deep breath.
Revealing mistake: In brother Justin's padded cell, the far wall is obviously a painted backdrop. It even flutters when the orderly and doctor walk in! The only solid part is a couple of feet at the end, that Justin leans up against.

Revealing mistake: After Beck has arrived at the coroner we see Margareta Oberg's body on a table. Looking closely you can see she's still breathing.
Revealing mistake: While Holmes and Watson are investigating the crime scene, there is a shot of them that shows the dead man's hand. His hand is visibly shaking, revealing the actor is struggling to maintain the position of his hand.
Tall Woman in Red Wagon - S1-E6
Revealing mistake: Harry Stoner is unconscious when Rockford and Sandra push him into the overhead couchette, but you can see his head raise independently from Rockford's sideways push shortly after he lands in it. (00:35:50)

Revealing mistake: Throughout the scene where Max interrogates Pierce, the projector changes slides at irregular intervals with no one touching it. (00:22:50)

Revealing mistake: When Lombard looks at Vera after realizing she stole his gun, the unruly wisps of hair are on his left temple, while in the rest of the scene they are on his right. Revealing mistake: During the big scene where the main Chucky is addressing the "Chucky army," you can tell most of the dolls aren't actually "alive." They're standing way too still and simply have a digital "blinking" animation applied over them.
Revealing mistake: As Sgt. Cain and Chief Lain stand by Marjorie's dead body lying on the hotel bed, discussing her death from an overdose, you can see her stomach rise and fall slightly, though she's supposed to be dead. (00:18:00 - 00:20:00)
