Duct Tape Canyon - S11-E8

Revealing mistake: When Jamie sits down after rappelling down the canyon wall you can see that he has climbing rope attached to his harness. It's wrapped in duct tape to look like its the rope he and Adam made of duct tape. This may just be a safety line to support his weight if he duct tape rope snaps, but the Mythbusters usually point out any backup safety systems and explain that they are there for caution and don't contribute to the test. (This was explained in the aftershow. It was a safety line required by the shows insurers).

No Trace of Tracy - S1-E4

Revealing mistake: Creek reads a newspaper article about Tracy, a missing schoolgirl. The first paragraph of the article is about the schoolgirl, but the rest is about John Major (who had just been ousted by Tony Blair as prime minister when the episode was first aired, but, assuming this was filmed some months earlier, would still have been in power). The first line of the article following the first paragraph starts mid-sentence, as if the first paragraph has simply been pasted over the top of the existing article. This mistake is surprising as there is a photograph of John Major alongside the article and the newspaper remains in shot long enough for it to be spotted without the need to pause the DVD or video.

Revealing mistake: In the arcade the killer talks to his helper while playing a pinball machine. He plays very agitated and strongly. But if you look closer at the below end of the playfield you see that there is no ball and the flippers aren't moving. (00:23:50 - 00:24:48)
