Audio problem: When Laura and Nellie are on the branch going down the rapids they can be heard talking, but their mouths either don't match with what they are saying or don't move at all.
Audio problem: After the mayor tells Andy that his three daughters will be in The Beauty Contest, Andy phones Ellie and when she hangs up on him we hear Andy say, "Hello? Hello?" but we see him only mouthing the second hello.

Audio problem: When Samantha and Darrin are at his ex-girlfriend's house for dinner, Samantha is doing all kinds of things to sabotage the dinner, even making a gust of wind come through the front door thus blowing the ex's wig off. The ex girlfriend is screaming but the screams don't match up to what her mouth is doing.
Audio problem: At the end of this episode, Aang tells Katara that forgiveness is the first step you need to begin healing. But if you watch closely, his mouth isn't moving at all when he says this.

Audio problem: When the Decepticon jets attack the maharaja's palace at the beginning Thundercracker speaks with Skywarp's voice. Later, at their base when the fake Optimus comes out Skywarp speaks with Starscream's voice.
Totally Spaced Out / The Switch Glitch - S2-E8
Audio problem: In Fairly Odd Parents, "Switch Glitch", Vicky forced Timmy to do her chores and homework, and stretched a dress on Timmy. He said "I can't do chores in a dress.". Vicky, however, was recording Timmy's voice on a tape, the "in a dress" part, and played the tape, but the tone of voice for "in a dress." was normal, not angry. Shouldn't it be the same?
Audio problem: The computer's voice says "golf shoes" while the screen says "golf cleates".
Audio problem: When Violet is singing in front of the audience at the final Glee Club performance, her voice does not match her lips.

Audio problem: In "Mommy Fearest", Ima Goodelady asks the Powerpuff Girls where they are going. Blossom tells her they're fighting crime, Buttercup speaks and then Bubbles adds, "Duh." However, her mouth doesn't move.
