Revealing mistake: (Part 4) After Omega takes off his mask and looks in the mirror, when he sees that his physical body doesn't exist he puts his mask back on and screams, but when he shouts, "If I exist only by my will, then my will is to destroy," Omega's mask pops up off his face, and we can see the actor's blackened face as he shouts the last bit of his dialogue. Revealing mistake: When the Capri crashes into the goal net & they go to arrest the suspect you can see sky satellite dishes on the houses in front of them. Satellite TV didn't start in the UK until the 80s.
Revealing mistake: Early in the episode the dead body in the barn ever so briefly flutters his eyelids.
Revealing mistake: There are numerous times throughout the series where the supposed big city of Metropolis is actually just a back lot set and none of the buildings are more than a couple of storeys high. Quite often the green hills of Southern California can be seen beyond the set and between the buildings.
Episode Five - S1-E5
Revealing mistake: The mirror over Barnabas' mantel hangs flush against the wall. Yet when he shatters it, the candlestick he throws can be seen to go through the glass instead of bouncing back as it should have done. (00:43:45)

Revealing mistake: When Flint follows the Indians, there's a strong wind blowing. But the two puffy clouds in the backdrop sky never move.
Revealing mistake: Season 5 Episode 18 "Spark of Life": when they show a closeup of the badly-burned woman during her debridement, it can be seen that even though her eyebrows and hair and, in fact, 90% of her skin has been burned away, she still has long, full eyelashes.
Revealing mistake: Before Jason Dean is shot outside the penthouse, Phoebe is waving her hands trying to convince him that the world is not real and you can already see blood on her fingers.
Revealing mistake: 1967: Barnabas and Vicky are talking in Collinwood's foyer when a great deal of noise starts interrupting the scene from backstage. You can hear coughing, several bumps and crashes, shouts, and finally, a fire extinguisher going off. Meanwhile, the distracted actors are completely blowing all their lines - but the unstoppable taping goes on.
Revealing mistake: Shortly after the fight with Becky, Roseanne asks Jackie to go across the street to get her something to drink, so she can smoke in secret. After Roseanne lights the cigarette and walks near the register, Jackie's plaid coat can be seen in the door's window as she waits for the next scene.
The Prodigal Son (1 & 2) - S2-E1
Revealing mistake: After Crockett and Tubbs' confrontation with the New York police at the end of part 1, as the police car drives away, in the background a blue car drives by and a man and woman can be seen in the car looking and pointing at the filming.
Revealing mistake: When Victor is opening the padlock, sparks fly around in close ups. In the wide angle shots there are no sparks at all.
Bio-Rhythms - S2-E19
Revealing mistake: In the multi-car collision, the ramp attached to the rear of the blue car to assist cars in flipping is visible. (00:04:10)
A. B. and C. - S1-E3
Revealing mistake: In the fight at the end of the B scenario, Number 6 has a small problem. His pants split open in the back. Fortunately, they somehow manage to mend themselves again.
Revealing mistake: When they show the soldiers killed by the Dalek, in both the hallway outside the cage and the large room in weapons testing, they show some casings which were ejected from the soldiers' guns. The casings still have projectiles in them. If used, they would have been in magazines and would never fall out like they are shown.
