Other mistake: During Pacey's phone call to Joey at the end of the episode, Pacey lies about being on a fishing trip and mentions having caught a "91 pound marlin" which is a pretty big fish. This doesn't make sense for him to say that, as a fish that large would be noticably missing when he and Doug returned home. And they wouldn't have likely thrown a fish that large back into the sea. It would reveal he was lying if he returned home without the fish.

Other mistake: (Season 4, Episode 1) During the scene where the phone sex operator lady is having her voicebox operated on, you can see on at least three different occasions her eyes flinch and squint. Obviously, she was "awake" when this was filmed, but during a normal procedure such as this, she should be completely sedated and she would not be flinching her eyes.

Misgivings - S4-E10
Other mistake: When Chris and Snoop go to kill Bug's dad, they have their high powered nail gun once again, even after previously throwing it in the marsh with the guns. Bought a new one? Not likely as the reason for throwing it away in the first place was because the cop saw it. Buying an identical one would defeat the purpose of ditching it. (00:55:50)

Other mistake: When Harold goes to kill his father, we see his dad on the seat moving each time we hear the hammer, but if you look at his head, you don't see anyone hitting him at all.

Other mistake: In episode 1x7, Barb has a list of all the family members' names and birthdays. On the list, the oldest daughter is a listed as "Amanda Henrickson". Her name is Sarah Henrickson: it's the actress who portrays her who is called Amanda (Seyfried).

Replacements - S1-E4
Other mistake: When Heffron is taking his throw at darts, he states that he needs a "double seven". When he takes the throw, the dart goes into the treble eight (an earlier close-up confirms that the board has the standard Gamlin configuration), yet, from the reaction he gets, he's made the throw he was going for. (00:04:40)

Episode Four - S1-E4
Other mistake: Jake is sitting at the kitchen counter top and is watching TV whilst doing his homework. You can see his red pencil case labelled 'Jake Rowlings'. Many times throughout the series the parents have been recognised as Mr and Mrs Brockman - not Rowlings. The children also have been referred to as Karen Ben and Jake Brockman. (00:13:40)
Suggested correction: Throughout the series, the Brockmans (specifically Pete and Sue) seem very ashamed of their family. In the first episode they tell Jake to give people Grandad's address. It doesn't seem out of character at all for them to tell Jake to give people a false surname. Also, Harry Potter came out at the same time so Rowling would be a very easy surname to choose. This isn't out of character for the parents at all if you look at the rest of the series.
The family's surname is not mentioned (other than on the pencil case) at any point in the first series. Likewise, the grandad is not named in the first series either. Sue even says she doesn't know anyone called Frank Morrison during Series 1 when Pete mentions a colleague of that name. This creates continuity errors when the family are given the name Brockman and Sue's father is established to be called Frank Morrison from Series 2 onwards.

Provenance - S1-E19
Other mistake: In the scene where Sam, Dean and Sarah go to compare the painting to the earlier photo of it, Dean, noticing the changed position of the razor, says, "Jared, check it out." He is speaking to the character Sam. The actor's name is Jared. [Confirmed in cast interviews].

Other mistake: In Season 2, Episode 14, "Double Negative" during the scene at the fundraiser, Kitty arrives and asks what the quickest way to a glass of wine is, to which Sarah (who is standing opposite her) offers her her own drink. Calista Flockhart (Kitty) does not see this, and another cast member attempts to take it from her, but Flockhart finally notices it and smirks at the realisation that she messed up.

Other mistake: When Cooper is on the floor after being shot, as he's signing the check, he is doing so with the pen closed, and proceeds to click it open as he hands it back to the room service.

First Blood - S1-E1
Other mistake: When he and Chloe encounter a group of lions scouring for food, what tips Oz off that something is wrong is the unusual sight of more than two males together. The more obvious indicator is that they are all male; in lion prides under normal circumstances, the hunting and gathering of food is done by the females.

Other mistake: The team, guns up, aimed, and ready, interrupt the meeting on the rooftop which has just ended in a shootout. Steve tells Cynthia to put the gun down or they'll have to put her down, and Junior menacingly cocks his rifle as if to emphasise the point. What, it wasn't already ready to fire? He's walked into a potential firefight with his weapon raised but it wasn't actually fireable yet?

Other mistake: Near the end of the episode, Liv Tyler gets into a bathtub and takes off her clothes. We see an overhead shot of the tub and it's clear she is wearing a darker colored top.

Other mistake: The wolf is accused of killing two officers, but they were only tased.
Suggested correction: "The wolf is accused of killing two officers, but they were only tased." It's actually he killed the 2 men not the officers thy were talking about the 2 men he had to help him.

Other mistake: The Lakota is attacking a small group of white men. One is face down on the ground, and he lifts his head to cut his throat. In the background are two cowboy actors joking and twirling their guns. One drops their gun and picks it back up. (00:20:43)

Other mistake: The CSI technician dusting for fingerprints on the gun isn't wearing gloves. He proceeds to pick up the gun in his, ungloved hands and hand it to an ungloved police officer thereby adding their fingerprints to the gun. Very sloppy police work.

Other mistake: Before the security guard is killed, he turns around and faces the direction of the camera. He is killed, and the camera backs up to reveal 6 men standing there. The security guard would have seen the men and he had time to react to them, but did nothing. (00:32:50)

The World Needs Heroes. Over. - S2-E9
Other mistake: The Doctor, coming up from the basement of his house, is locking both deadbolt locks using a key held in his right hand. He logically would use the same hand to put the key in his upper left pocket. Richard who is watching through binoculars uses a walkie talkie to inform both Ezra and Jules that the Doctor has put the keys in his upper left pocket. However, the next scene shows the Doctor with keys now in his left hand placing the keys in his upper right pocket. This doesn't match what Richard says he observed. (00:24:42)

Mona Lisa's Revenge (1) - S3-E10
Other mistake: Mr. Smith scans the Mona Lisa and determines that it is genuine from DaVinci's brush strokes. He should have also noticed the words "This is a fake" written in felt tip under the paint, from when The Doctor replaced the Mona Lisa with one of DaVinci's "forgeries" in "City of Death."

Chapter Sixty-Eight: Quiz Show - S4-E11
Other mistake: Archie's uncle Frank tells Archie during an argument with him that he is 40 years old, with seven years in combat while in the military. But in a previous episode when explaining why he let Fred take the blame for his DUI, he claimed that he was a Desert Storm veteran in between tours at the time. Either the show got the wars wrong and meant to say he was an Operation Enduring Freedom or Iraqi Freedom veteran (no actual veteran uses "Desert Storm" when referring to combat in the Middle East after 9/11) or they're saying that he was only 10-11 years old when he went to Desert Storm. (00:29:30)
