Continuity mistake: At the end of the battle between Jaden Yuki and Gravekeeper's Chief, just as the explosion settles, in one shot you see the Gravekeeper's Spear card to the left of Gravekeeper's Chief on the duel disk. Continuity mistake: When Roberto is looking at the team having a laugh with the still injured Ishizaki, there are 10 of them in the first shot with no keeper included and 10 including the keeper in the second. Both are wrong though, since Ryo has a substitute that is playing, so should be 11 players all along. Later in the episode when they run for the special training, it's 12 people, with Ishizaki still sidelined, but in the close-up of Roberto's foot you can see them run past, and it's 11 players. Visible crew/equipment: In the scene where Mcduff and Johnson are trying to find water since it's extremely hot, they go Under The Bed and notice Alfred under there. Look in the background, for just a split second, you can a man walk past the from the door. It's for only a split second so it's quite easy to miss. (00:06:05)

Iron Forged in Fire: Part One - S1-E1
Continuity mistake: Tony walks by some people in the Stark Lounge, and from a high angle their shadows are seen. The shadows are very close to the people’s feet, showing the light source is almost totally overhead. A couple shots later, you see behind Tony as he is walking, the same people in almost the same spot, with their shadows extended out further, as if the light source has moved several feet in front of them, but while they are still inside the building. Continuity mistake: Perry White tells Clark and Lois that The Mad Scientist will strike tonight and then puts his hands on the letter from the scientist. It cuts to a close up of the letter as he begins to turn it. Between the shots, his right hand fingers go from touching the word "Heed" on the letter to being an inch or so below the word, and the fingers on his left hand are suddenly much closer to the paper. Plot hole: White Base and the interior of Luna II withstand without a scratch the explosion of the thermonuclear reactor from the Magellan from just a few meters, while the blast literally vaporizes the Zaku far away from the entrance and threatens to damage the Musai, hundreds of meters away.
Other mistake: While Sonic and Tails are locked up in old Spelunk's cage during the cave-in, right before the huge diamondius crashes on top of the cage, there's a close-up of Sonic holding the bars, and the color of his torso becomes the same as the background mid-shot.
