Continuity mistake: In the fight scene at the start of the episode, Jesse knocks one of the GS agents into the water - in subsequent shots he has disappeared.
Continuity mistake: The packages Drake and Kathy place on the table beside their coats have disappeared when they return for the coats a few shots later. (00:19:00 - 00:21:20)
Continuity mistake: When they first enter the Y'Lyntian city and are attacked by the Lava Wraiths, Raphael gets his sai melted by them. But after taking their goodbyes from Versallia, Raphael has his sai stuck back in his belt.
Continuity mistake: Elisabeth Shue is talking to Homelander by her car. As she gets her baby out of the car, she says "can we please just talk later" and has a muslin hanging over her right shoulder, turns around, and from the reverse angle the muslin is bundled up in front of her, not over her shoulder at all. Then the angle changes again and it's back where it was before.
Fear Burns Down to Ashes - S4-E15
Continuity mistake: Rev Bem returns for this episode, however the costume department seems to have forgotten all the changes his character went through in S3 "What Happens to a Rev Deferred?" His face is distinctly more Magog than it should be, and his hands have reverted back into clawed fingers.
Continuity mistake: When Agatha is talking to Herbert at the police line, the position of her arms varies over several shots.
Continuity mistake: Gar enters a room and leaves the door open. Throughout the whole mirror scene the door is also open both when the camera looks at him and when the camera points at the mirror. However, when he is about to leave the room he needs to open the door as it has been closed inbetween us looking at Gar in the mirror and looking at him normally. (00:17:50 - 00:18:35)

Continuity mistake: Kendall is wearing black heeled boots when she is fighting the aliens, switches to black sneakers when she lands after being thrown and has the boots again when she stands back up.
Battle Scars - S1-E7
Continuity mistake: When Echo prepares to calibrate the surgical pod, he has a bandage on his head, even though the members of the Bad Batch don't get their control chips removed until near the end of the episode. (00:14:11)
The Crab with the Golden Claws: Part 1 - S1-E1
Continuity mistake: When the three bad guys make for the car after taking Mr. Kuraki with them, two of them are headed for the right hand side and the bad guy with the purple jumper runs to the left side. In the next shot the bad guy with the purple jumper is now on the right hand side of the car, pulling the door shut. (00:06:45)
Continuity mistake: Just before they leave for the meeting at Lottie's Place, Jason places a hand on Jeremy's shoulder. When the shot cuts, his hand jumps to a much lower position on Jeremy's forearm. (00:41:05)
Triumvirate of Terror! - S3-E8
Continuity mistake: Right at the beginning, the camera closes in on a baseball field. It shows several people standing and walking out in the field and just two standing up by home plate. The view changes to behind home plate and suddenly there are several superheroes sitting and stretching there, and the rest of the field is empty. Then after a shot of Jimmy Olson in the commentator stand, it shows a high view of the field again and the people have all changed again. (00:00:10)
Continuity mistake: In her rooftop fight with Tommy, Batwoman opens with a batarang, then sends him face down kicking him from behind. They wrestle some, and the railgun goes off once. In a wide shot when she punches him in the midsection a few times while he is trapped in an armlock, you can see the gun on the ground. He had it in hand in the previous shot and is still in his possession later. (00:35:00)
Iron Forged in Fire: Part One - S1-E1
Continuity mistake: After the plane explosion, a scene six months later is shown with Tony lying in bed with his chest piece now in that keeps him alive. The first shot shows it with some wire or designs of some sort protruding around it along his chest. Some of the ends overlap with the border of the central round piece. It then cuts to a closer view of the device as Tony’s alarm sounds, and suddenly the pieces do not overlap the circle at all, and some of them slightly change shape and position. (00:06:35)
The Incomparable Connie Walker - S1-E18
Continuity mistake: "The Incomparable Connie Walker": When Dillon visits Mayor Walker's house, Walker's red carnation is on his left lapel. But one shot of him is, for some reason, reversed, causing the carnation to switch itself to his right lapel for a moment.
Continuity mistake: At the beginning when the lady at check in is passed out on the floor, she is right up against the desk, but when Lois goes behind to tend to her, she is further away from the desk, and closer to the wall.
Weirdmageddon: Part 1 - S2-E18
Continuity mistake: Two guys are throwing a frisbee and it lands on the ground. In the next shot, the frisbee is gone.
Yomigaere! Eiyuu Densetsu - S1-E1
Continuity mistake: The Pegasus armor is contained by a box, metallic grey, placed on a slab next to the Grand Pope. When Seiya cuts off Cassios' ear, the box appears gold/bronze colored, and when he tells Cassios "I owe you quite a lot of payback", the pillar with the armor is right behind him already instead than the top of the stairs. Continuity mistake: One moment Frankie takes out two shot glasses and a bottle and starts to pour. She pours maybe one-fourth of one glass. The very next shot, both glasses are filled. (00:03:35)
