Continuity mistake: During the battle a railing on a pirate ship falls off, only to be seen in the next shot intact, then missing in a subsequent shot.
Continuity mistake: When Jennifer Walters is first introduced, her fingernails are unpolished. Later when she is restrained in Dr. Doom's lair, her fingernails alternate between being painted pink and red. Given that she and Bruce were running for their lives, she wouldn't have had time to paint her nails. Later when she transforms into She-Hulk, her fingernails turn green with the rest of her body, but if her fingernails were painted, they should still appear that color, as her skin and hair turning green wouldn't magically remove nail polish.
Continuity mistake: When Flash plays tag with Faust ("Now you see me, now you don't"), there is a brief shot in which he rushes at Faust - but not inside Hyppolita's palace, but in some high-tech laboratory. Obviously a shot from some earlier series, since the coloring and style are different from the Justice League's.
The Ringer - S1-E1
Continuity mistake: Zelda has an amateur magician grow a large plant for her to use as a sling shot. He is standing beside Link as it grows and then suddenly disappears between shots. There was not enough time for him to have walked or run away. (00:11:40)
How the Tess Was Won - August 5, 1956 - S1-E5
Continuity mistake: Though he's leaped into someone else, we always see Sam as Sam, dressed in all the host's clothes (including glasses or sunglasses). His host is revealed only in reflections. But here, when he looks at his host-self in the mirror, the hat and clothes are all identical - except that the reflection is wearing glasses, and Sam isn't. (00:43:00)
Continuity mistake: When Mando switches on the jetpack, the reaction shot on Cara Dune is flipped. In the same shot, Mando also has the jetpack mistakenly still in hand, but he already put it on his back. (00:38:15)
Continuity mistake: As they are fleeing the alien base, Supergirl and the others run towards the portal, Winn activates it and jumps through. In a following shot (after a 'friendly' alien helps them), he is standing behind Supergirl again. (00:32:00 - 00:32:35)
The Thanksgiving Story - S2-E10
Continuity mistake: Throughout the show Rebecca has long hair. Suddenly it's shoulder length when talking to Israel while he was in bed.
The System - S3-E15
Continuity mistake: Phelps takes the tape from the recorder's right-hand spindle. When he opens the acid vat, we can see the tape he just removed back on the recorder - but on the left spindle. The tape he then drops into the acid doesn't remotely resemble the one he pulled, and when he drops it, the recorder's left spindle, where the full tape was in the last shot and where the empty reel should be, now has nothing on it at all. Continuity mistake: Tenaya 7 is running on the treadmill in flat sandals with tan straps, she then has flat sandals with black straps as she fights the guards.
Continuity mistake: The first four episodes of season two have absolutely no continuity with the show. Prime examples are the fact that Shendu exists again as a statue which had been confirmed to be destroyed in "The Day of the Dragon", the final episode of season one. Next during "The Warrior Incarnate" it is mentioned that not all the talismans were in fact present, such as the dog talisman, when all talismans must have been found, or the events that escalated during the final episode of season one would never of happened. This problem is the extended during the next episode where Jackie Chan discovers the snake talisman in some sort of Brazilian temple - he had already learned of the snake talisman by stealing it from the museum quite early on in season one. The final continuity error is that Tohru goes from friend to foe and then back to friend as if the first four episodes of season two never happened.
Suggested correction: These were "flashback" episodes, not "they never happened" episodes.
Continuity mistake: When Raven telekinetically reassembles the broken chime bead necklace, it has about nine beads and is rather tight, but before and after that the necklaces feature about 13 or 14 beads and are wide enough to fit around the others' necks.
Labor Pains - S2-E8
Continuity mistake: At the beginning during the bridge jump scene, the A-Team van changes. It suddenly has round headlights, its hub caps can be seen popping off when it lands, and it doesn't have any spotlights. The real van has double square shaped headlights, steel spoked rims, and spotlights. (00:05:20)

Continuity mistake: T'Pol says she has never been camping, but she went in "Strange New World."
Continuity mistake: In the bootcamp scene where Captain Sobel is yelling at Private Christenson for drinking from his canteen, you see a machine gun resting on Christenson's right shoulder. When the camera angle changes to behind Christenson, the machine gun has suddenly moved onto his other shoulder. This happens quite a few times in this particular scene.
Rising (1) - S1-E1
Continuity mistake: When they first arrive on Teyla's planet, the DHD is close to the gate. When the wraith attack, the DHD is much further from the gate.
The Deadly Decoy Affair - S1-E15
Continuity mistake: In the intro, Illya gets into the car and as he's talking to Solo, finishes the last of his breakfast donut in close-up. When we cut to a two-shot, he finishes the donut all over again. (00:03:40)
Continuity mistake: During the car chase through the desert, the rear view mirrors on both cars disappear and reappear repeatedly.
The Feminum Mystique: Part 2 - S1-E6
Continuity mistake: When Wonder Woman is spinning the XPJ-1 around, she's seen approaching the jet with her hands empty and her lasso on her side where it usually is. As the scene progresses, her golden lasso magically appears in her right hand even though she never reached down to pick it up.
Continuity mistake: The six Rangers are standing together as Wolfbane is enlarged by Orbis. Kat and Tanya change places when switching from the wide shot, to a closeup of the Rangers.
