Continuity mistake: When Sherlock steps out of the taxi on Baker Street three extras walk by. First, it's a lady with her back to the camera, wearing a dark jacket, purse, boots and a dark curly hair. Next, the two women walking towards the camera. Notice the blonde one on the left with the pony tails, dark jacket with the multi colored cloth at the bottom, and grey pants. Fast forward to when the taxi pulls up to take Sherlock and Watson to the the crime scene. As Sherlock opens the taxi door, the same first lady with the curly hair and the blonde with the ponytail are now walking together in the same direction the blonde was walking when Sherlock arrived. Continuity mistake: Early in the episode Sherlock and Dr. Watson enter a cab in daylight for a ride to the crime scene in Brixton, which is a part of greater London. It is night when they arrive.
Continuity mistake: As Sherlock and Watson come out of their apartment to hail a taxi to be taken to Brixton and the scene of a murder, two extras can be seen in the shot - one crossing the road and one walking up the far side of the street. At the top of the street there are two white colored cars, but they suddenly disappear, to be replaced by a dark colored car. Also the light of the building across the road is suddenly turned on.
Continuity mistake: When Sherlock is backstage at the circus and finds the yellow spray paint, he draws a line on the mirror. In the next shot, the line he drew is completely different (and in fact is more of a squiggle). (01:07:40)
Continuity mistake: When Sherlock and Watson first get to the bank, the time for New York is 7:45, then when Sherlock is trying to find out who the message is for, the time for New York is 7:20.
Continuity mistake: In the opening scene when John is going to the refrigerator, a white skull can be seen on the left side of the kitchen table amongst all Sherlock's science equipment. When he closes the refrigerator door again after discovering the severed head, the skull has disappeared. As he's leaving the kitchen, the skull reappears. (00:03:25)
Continuity mistake: When Sherlock goes to look out the window at John leaving (after the tiff about the head in the fridge), his dressing gown is hanging off his right shoulder, exposing his t-shirt. Next there is a wider shot of him from behind, and his dressing gown is on straight, covering both shoulders properly. Then there's another closeup and it's off the right shoulder again. (00:05:40 - 00:06:10)
Continuity mistake: When Sherlock pushes the door open to leave the museum, it closes from the other side when reentering the frame. (01:00:56)
Continuity mistake: At the end of the episode when Sherlock takes the explosive jacket off John, he throws it away from them both. You can see the jacket change places during each camera shot. (01:27:10 - 01:28:02)
Continuity mistake: After receiving the third pip. Dr. Watson turns on the television to the news. During the news segment, it states the age of Connie Price as 48. Then in the scene immediately following, DI Lestrade says she was 54.
Continuity mistake: When Sherlock pulls the gun on Moriarty in the pool scene, he is holding it with two hands but then puts one hand down. In the next shot, he has two hands on the gun again. Then again in the next shot he only has one hand on the gun.
A Scandal in Belgravia - S2-E1
Continuity mistake: At Buckingham Palace, Watson has just sat down next to Sherlock. There is a red pillow between then on the couch. It moves, sometimes sitting straight and sometimes tilted, between shots throughout the scene. (00:14:45)
A Scandal in Belgravia - S2-E1
Continuity mistake: We see John come into the room and stand next to Mrs. Hudson and put on his coat. After a few shots Mrs. Hudson walks away with the plates and says, "Lovely tune, Sherlock..." As she's walking away we see John isn't wearing his coat, and he puts it on again. (00:50:00)
A Scandal in Belgravia - S2-E1
Continuity mistake: Near the end, when Mycroft is talking to Watson, he folds his hands and places his head on them. However, in the next shot, his hands are a couple inches away from his face.
Continuity mistake: When Moriarty puts the gum on the glass case around the crown jewels, the gum moves between shots.
Continuity mistake: In the scene where Sherlock is on the roof of the hospital with Moriarty, when Moriarty shoots himself in the mouth, when the camera shot quickly changes from a close up and Sherlock moves back, the gun disappears from Moriarty's hand (to the right of Sherlock). In the next shot of him, it's still in his hand.
Continuity mistake: As Sherlock and John approach the missing subway car, and especially as they bend to peer beneath it, the interior of the car to a distance of several feet is plainly visible through the window of the door (you can see the curved ceiling inside and some unlit fluorescent light bars). However, an interior shot then shows that there is a closed door inside the car, a foot or so inside the exterior door, that Sherlock has to push open to enter the car proper. This door should have blocked their view of the inside of the car before they entered. It didn't, so it has gone from open to closed between shots without anyone touching it. (01:05:35)
Continuity mistake: The underground car seen in the CCTV video is from the Jubilee line, but is meant to be the District Line. It's also different from the one where the heroes defuse the bomb, which is an actual District Line train (D stock). The rounded doors is the most visible proof.
Continuity mistake: The scene where Sherlock says "Vatican Cameos" he is seen to be facing the right side of the shot. Yet in the shot before that he is facing the left side of the shot from the back.
Continuity mistake: Sherlock is dancing with the bridesmaid towards the end. When they stop and are talking with Watson, Sherlock's hands keep switching from being behind him to in front of him, depending on the camera angle.
Chosen answer: That line of dialogue has never actually been spoken in any of the Sherlock episodes, during seasons 1-4. However, that line is written in someone's fan-fiction story online, where it's said by Sherlock and directed at Anderson.
