Continuity mistake: In Room 210, Newkirk's sleeves are down to his wrists, but in the close up of his hands, the sleeves are totally missing.
Visible crew/equipment: As Hogan, Le Beau, Kinch, and Schultz talk at the car, the boom mic is reflected in the chrome and on the hood.
Revealing mistake: The bunk that is the entrance to the tunnels appears to be moving upward about a split second before Newkirk taps the hidden key to activate it.
Other mistake: Hogan is disarming a booby trap briefcase. The bomb expert tells him over the radio he has to decide to turn the handle clockwise or counterclockwise. The expert doesn't know which way to turn it. Hogan turns the handle counterclockwise and disarms the booby trap, and the expert says counterclockwise. There's no way the expert would have known which way Hogan turned the handle. (00:21:50 - 00:22:26)
Other mistake: As the boys are unloading the truck, one of the cases is WEIN (wine) , and marked with a 'Z' in the corner. In Season 4 Episode 22 "The Purchasing Plan", the boxes marked "Z" were boxes containing nitro glycerin for the underground.
Continuity mistake: In this episode, the picture next to the light switch wire is a group seated around a table. In "The Defector" (S5:Ep10) the picture was of Hitler making a speech, and the microphone he's speaking into hides the Heroes' bug.
Continuity mistake: As the men are lined up, Klink starts to grill Hogan. Klink has his pistol aimed directly at Hogan, then it's at a 35 degree angle toward and close to Hogan's face. A moment later it's about two feet away from his face.
Continuity mistake: Toward the end of the episode, Hogan turns Shultz around, grabs one of the grenades Shultz is carrying, and hands it off. The one grabbed was closest to Shultz's left side. A few moments later, Shultz turns his back briefly to the camera; there is now a space between the grenades, as if Hogan grabbed the middle one.

Character mistake: Schultz states "In Hammelburg, they call me die Glitterzehe", which is a literal (but wrong) translation of the term "twinkle toes." That term does not exist in German, and even if it did, it would have to be GlitZerzehe, because the German word for twinkling is 'glitzern', the word 'glittern' does not exist.

Visible crew/equipment: When Burkhalter first comes into the office, the camera rolls a little closer. As it does, you can see the shadow of the camera on Burkhalter's arm.
Continuity mistake: As Schultz takes the balloons away from Hogan, Newkirk is nowhere to be seen. A second later, he witnesses the entire scene.
Continuity mistake: As Klink asks about the forecast, his hand is around the microphone end of the receiver, then a moment later it's around the handle.
Audio problem: When Klink comes into the barracks, he puts the bottle of alcohol into his jacket to hide it. When he does, you hear a distinct clink (no pun intended) of glass on metal. None of the POWs wear anything that would cause this.
Continuity mistake: Before Hogan lets Klink have the ball to hit, the camera does a close up on the volleyball. It can be seen to be a dull light brown color. After Klink hits it, and it soars up into the air the camera cuts to Hogan and Klink, and back again. When it does the ball is now a shiny orange like color.
Continuity mistake: After Hogan calls off the volleyball game to go see what Kinch wants, he goes into the barracks. From the outside, the door is already open. When the camera shot is indoors, Hogan is opening the door again.
Continuity mistake: When Shultz comes out of the NCO Club, he bumps into Klink. After yelling for a moment, Shultz talks with Klink. While talking, Klink has his monocle on. After taking a couple of steps past Shultz, he turns around and addresses Shultz again; when he does so, you see that the monocle is missing, without being taken off.
Continuity mistake: When out at the volleyball net, Hogan is called by Kinch. While walking in, Hogan's hat is positioned as normal. When the shot is inside, the bill of his hat is down shading his eyes, and he pushes it up into its regular position.
Factual error: In the rec hall where Klink is weightlifting, there is an advertisement poster for the Grindelwald region on the wall, which is in Switzerland. The German propaganda poured a lot of money into promoting the Reich's own recreational regions through the "Kraft durch Freude" program. With the war going badly, they especially wouldn't want a "Look how beautiful Switzerland is" poster in plain view of their troops every day.
Visible crew/equipment: As Klink and Schultz approach the barracks where Le Beau is "weightlifting", there is a cord or cable along the ground that would be for studio equipment.
Continuity mistake: When the woman is kissing Klink in town, she hands a note to the boy who puts the note inside the spare tire's hubcap. There was no tape to stick it anywhere, yet when LeBeau goes to get the note, it is stuck with tape to the center of the spare tire.
