Continuity mistake: When Dean is speaking to Sam, who is now Lucifer, he starts crying. Tears are physically on his face but in the next shot the tears are gone.
Continuity mistake: In the scene where Dean uses the silver knife to cut his arm to prove he is not a shape shifter, a few moments later Dean is showing Bobby the hand print on his arm, and the cut is miraculously gone. (00:13:00 - 00:15:00)
Visible crew/equipment: At the start of the episode, the couple comes out of the car and when it pans to show the orchard, a shadow can be seen walking in the lights.
Continuity mistake: When Sam and Dean are tied up in the sewer, Dean gets out of the ropes holding him and the camera changes to Sam, but in the background you can see Dean with the ropes still around his arms & upper body.
Visible crew/equipment: When Dean thanks the professor before leaving the room, you can see the second camera pan down on the left side of the screen before it cuts to that camera's view. (00:28:40)

Factual error: Officer Kathleen looks for Sam's data in her database. The record ID is DF-23094. She then follows the link to Dean's record. Its record ID is also DF-23094. A database cannot have two records with the same ID.
A Very Supernatural Christmas - S3-E8
Revealing mistake: When the boys are tied up and the pagan gods are preparing for the ritual, the second time they cut into Dean's arm the knife is pressed along his arm and the blood is removed showing no cut at all.
All Hell Breaks Loose: Part 2 - S2-E22
Revealing mistake: Sam is breathing noticeably at the beginning of the show on the bed even though he is supposed to be dead.
Fresh Blood - S3-E7
Revealing mistake: Sam takes the SIM card out of his phone so it can't be tracked, but when he takes the back off his phone, there's no battery, and the SIM card is just lying in the back not attached to anything! Same for Dean's - he takes the back off and the card just falls right out, not secured in any way as it would be in a real phone.
Visible crew/equipment: When Sam tells the little girl to take her brother outside. The little girl runs out with her brother and Dean asks the little girl where Sam is, Dean runs to the Impala and grabs a shotgun. When Dean is running to the house, he runs on the lawn and you can see the shadow of the camera man running with Dean at the bottom of the screen.
Playthings - S2-E11
Continuity mistake: In one scene Sam is hung over the toilet from drinking too much with his cast on his left arm. The rest of the show the cast is on his right arm. (00:20:19)
Fan Fiction - S10-E5
Plot hole: Marie says at the beginning that Chuck quit writing after Swan Song (season 5). Later she says she hates the "Metastories" which don't happen until season 9. She couldn't have known about the Metastories since those books were not published.
Suggested correction: I was mistaken about what a 'meta-story' is when I wrote this. A meta-story is when the characters refer to their own fictional state. I thought it was referring to the Metatron stories.
Continuity mistake: Dean drives the Impala to the coroner's house at the same time Sam is driving Castiel and Jack back to the bunker, also in the Impala. (00:31:15 - 00:33:45)
Continuity mistake: Gary (disguised as Sam) sits at the bar and orders a banana daiquiri. The woman next to him tries to engage him in a conversation as he sips his drink, and each time the camera switches between him and the woman, the level of his drink changes noticeably up and down.
Revealing mistake: During the love scene between Dean and Cassie, you can see the tape used on Cassie to keep the show PG-13. (00:23:15)
A Very Supernatural Christmas - S3-E8
Continuity mistake: In 'A Very Supernatural Christmas' (S3E08), when the Gods are beginning to sacrifice Sam and Dean, they slice Sam's right forearm. However, at The End of the episode, Sam has his left forearm bandaged, not his right.
Continuity mistake: When the Carlton son reaches in to pull the plug out of the sink you can see the plug chain on the cutting board to the left of the sink before he even reaches in, then he pulls it out and puts the plug chain on the board. (00:14:30)
All Hell Breaks Loose: Part 2 - S2-E22
Continuity mistake: When Dean kisses the Crossroads Demon, his head is tilted left, but when it cuts to the next shot his head is tilted to the right. It switches back and forth in the next two shots too quickly for Dean or the demon to move their heads.
Continuity mistake: During the scene where Sam is tied up and eventually fights the fake-Dean in Becka's house, there is a shot that shows both his feet and hands tied. During the fight, both his hands and feet are shown free, but he only cut free his hands, and began fighting immediately after that. (00:25:20 - 00:25:55)
